Job summary
We are looking for an experienced Practice Nurse who can deal with a range of long term conditions as well as the general day to day nursing care of our patients. You will be supported by an established clinical team and there may be opportunities to expand your scope of practice.
Our working week is 08.00am-18.30pm Monday - Friday, plus Saturday mornings on a rota basis.
Main duties of the job
Responsible for the delivery of basic practice nursing services to the whole practice population. Supported by senior nurses within the practice, to deliver care within the boundaries of the role, focusing upon health promotion, health prevention, immunisation and screening.
To work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
About us
We are a Training Practice with a patient list size of approximately 10,000. Our Practice team consists of 3 GP Partners who are supported by 13 clinicians, 12 reception and administration staff and a practice management team of 3.
We have one site in Hawkinge which is currently beingexpandedand we have one rural site in Elham.

Clinical
- Assess, plan, develop implement and evaluate treatment programs that promote health and well- being.
- Proactively identify, and manage as appropriate treatment plans for patients at risk of developing a long term condition.
