Are you a pharmacist looking to join an existing team of friendly and supportive PCN clinical pharmacists?
South Norfolk Health Improvement Partnership (SNhIP) PCN is looking to recruit a further Clinical Pharmacist to join their established pharmacy team. Full and part-time applicants are welcomed for this role and compressed full-time hours will also be considered. This role is primarily based within our PCN surgeries, however there will be some opportunity for remote working.
Main duties of the job
As a PCN we are committed to providing an excellent level of healthcare to our population, and have already begun building a diverse and talented primary care team across the PCN. We recognise the huge benefit clinical pharmacists provide and are committed to finding the right pharmacist(s) to help the patients registered within our PCN practices.
Clinical Pharmacists within SNhIP PCN are expected to:
- Work as part of a diverse multidisciplinary team in a patient-facing role
- Undertake structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy in our care home population and wider community
- Support medication monitoring within the practices
- Complete medicines reconciliations following transfer of care e.g. hospital discharge
- Create strong working relationships with our care homes and support them with medicines management and medication queries
- Support attainment of the QOF, PCN DES and IIF
- Respond to patient queries or concerns around their medications
- Work with prescribers to promote safe, effective prescribing and deliver Quality Improvement (QI) initiatives
- Run chronic disease clinics within your area of competence
- Conduct clinical audits and feedback results to relevant parties
- Support education of other staff members in the PCN and visiting healthcare students
About us
SNhIP PCN is made up of 9 practices in South Norfolk. Our nine practices are located in a mix of rural/semi-urban areas representing around 83,000 patients. Our practices include: Attleborough Surgeries, Chet Valley Surgery (Loddon), Church Hill Surgery (Pulham Market), Harleston Medical Practice, Heathgate Medical Practice (Poringland), Old Mill & Millgates Medical Practice (Poringland), Long Stratton Medical Partnership, Parish Fields Surgery (Diss) and The Lawns Surgery (Diss). Our clinicians are proactive and work collaboratively together whilst still maintaining our individual practice identities.
The pharmacy team within the PCN are responsible for provision of clinical pharmacy services across the member practices. At present, pharmacists work with a select number of surgeries across our PCN which helps to create strong and supportive professional relationships with the other members of the healthcare team. The successful applicant would be expected to travel across the PCN to provide clinical pharmacy services to the practices.
Clinical pharmacists joining SNhIP PCN will be supported on their CPPE Primary Care Pharmacy Education Pathway (PCPEP) and Independent Prescribing qualification, if not already completed. Continued professional development (CPD) opportunities will also be supported.
