Job summary

Long Stratton Medical Partnership is looking for 2 enthusiastic and highly motivated Advanced Nurse Practitioners to join our 11300 patient friendly rural practice. We have a very supportive team committed to high quality patient care with a strong ethos on team working and integrated care.

Ideally we are looking for 2 ANP's to cover 5 days Monday to Friday, however we have a degree of flexibility and will consider applications for part time working from the right candidates

Main duties of the job

You will need to be a degree qualified Advanced Nurse Practitioner wit relevant clinical skills and expertise in order to work autonomously to help provide ongoing clinical care and manage acute demand.

You must have relevant experience and be a Nurse Prescriber enabling you to independently assess, diagnose, treat, and refer patients as appropriate

You will manage your own patient booked list on a daily basis as well as potentially taking on specific responsibilities in other areas of the Practice

About us

You will be part of a team of 8 GP's, chronic disease nurse team , HCA's and PCN staff (Mental Health, First Contact Physio, and Clinical Pharmacist) as well as an experienced administration and management team.

We are a GP Training and UEA Medical Student Practice that puts patient care first and endeavours for continuous improvement.

Job Summary

1. The ANP will act as an autonomous, accountable practitioner working on their own initiative and in collaboration with nursing, medical and allied health professionals to assess individual patient needs, initiate investigations, determine a differential diagnosis and initiate appropriate holistic evidence based treatment and care.

2. The ANP will be responsible for the provision of effective, efficient and appropriate high quality care and act at all times in accordance with the NMC Professional Code of Conduct within their level of competence and agreed protocols.

3. The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

Key Responsibilities

Clinical Practice

The ANP will be expected to conduct their clinical practice within the surgery setting or whilst visiting patients at their place of residence. Furthermore, the post-holder will participate in the Practice rota for Duty Clinician. The key responsibilities for this role are:

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and Practice protocols, and within scope of practice
  • Initiates appropriate diagnostic strategies and therapeutic interventions including radiology and laboratory requests, with consideration and attention given to safety, acceptability and efficacy
  • Interprets and evaluates diagnostic test results in liaison with the patient, carers, family and their GP
  • Work with patients in order to support compliance with and adherence to prescribed treatments
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
  • Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation
  • Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
  • Support the Practice with the organisation and implementation of the seasonal influenza and Pneumococcal vaccination programme
  • Implement and participate in vaccination and immunisation programmes for both adults and children
  • Advise, support and administer vaccinations for patients travelling abroad, where appropriate
  • Promote and deliver evidence-based care for patients presenting with aural conditions
  • Meet the needs of patients presenting for opportunistic wound care
