Job summary
We are looking for an experienced and proactive Practice Nurse to join our established practice nursing team.
We would be looking for you to work approximately 30 hours a week.
The successful candidate would need to be motivated and committed to providing high quality care and work collaboratively with the general practice team to meet the needs of our patients.

Main duties of the job
Your Duties will include:
Running Nurse led clinics
Chronic Disease Management (Asthma, COPD, Diabetes)
Health promotion and Lifestyle advice,
Cervical Screening,
Immunisations and Vaccinations,
Contraceptive Services,
Supporting QOF and Primary Care Standards priorities,
Maintaining accurate clinical records,
Infection prevention and control,

About us
Located just outside the heart of Wigan town centre, The Dicconson Group Practice is a well established GP Practice serving a diverse patient population of approximately 9650 patients.
You will be joining a welcoming and experienced clinical team at the Practice.
This is headed up by 4 supportive GP Partners. We have a dedicated team of 3 salaried GP's . 2 Advanced Practitioners, 3 Practice Nurses, a Nurse Associate and our Healthcare Associate.
They in turn are supported by our non-clinical team, which includes our friendly reception team , 2 dedicated secretaries and data quality lead as well as our management team of 3.

Management of Chronic Diseases which includes asthma , hypertension, CHD, Diabetes, COPD,

Administer adult and children's Immunisations and vaccinations,
Contraceptive Services

Maintaining accurate documentation of care and utilisation of IT
