Job summary
We are looking for someone to undertake this role for 12 months maternity cover, working as part of our Hospital in reach service to homeless in patients at the MRI.
Urban Village Medical Practice deliver a Homeless Healthcare Service which provides accessible comprehensive primary care offer to homeless people in the City.
As part of this service, consisting of clinical and non clinical staff, we operate a hospital in reach service in the MRI in order to identify homeless
inpatients and undertake effective discharge planning in collaboration with
secondary care colleagues with a focus on engagement with appropriate primary
care following discharge. This includes a documented assessment of needs of
the patient, and carrying out identified actions as required to achieve
engagement in primary care post discharge.
This is fast paced role, working in a busy and changing environment.
Main duties of the job
The ideal candidate with have up to date knowledge of housing entitlements, strong knowledge of the local support offer for homeless people, and excellent organisational and admin skills with experience of working with people from marginalised and disadvantaged groups.
The postholder will need to be able to work under pressure and in a fast paced environment, working alongside clinical colleagues to deliver services, sometimes under tight timescales.

Our Homeless Healthcare Service is part of the wider general practice team at Urban Village Medical Practice and has experienced clinical and non clinical team members. We are a well respected team both locally and nationally. We offer accessible primary care both at the Practice and in a range of outreach locations, and also offer a hospital in reach service at the MRI. Our team works closely with a range of partners delivering services for homeless people across the city including statutory and voluntary services.

- GCSE grade A-C or equivalent in English & Maths
- NVQ level 2/3 or equivalent experience
Desirable
- A relevant qualifications in health, social care or other related subject.

- Demonstrable experience of working with people who are disadvantaged or socially excluded
- Experience of multi-disciplinary working to identify and implement outcomes for clients
- Experience of assessing need and working within time-limited interventions to promote and support change.
Desirable
- Experience of direct work with homeless people or people with substance misuse issues
- - Experience of working in a healthcare setting
Knowledge and Skills
Essential
- A sound knowledge of local support agencies and a commitment to the continual updating of personal knowledge in this area.
- An up to date knowledge of housing entitlements
- Excellent communication skills across a range of sectors, both verbally and in writing.
- Excellent negotiating and influence skills
- IT literate, with a sound working knowledge of Microsoft packages and data management/case recording systems.
