Job summary

We are looking for a motivated experienced Advanced Nurse Practitioner to join our team at Guiseley and Yeadon Medical Practice. The role will involve sessions carrying out appointments reviewing acute and chronic health problems and working with our frailty team carrying out care home and home visits, carrying out frailty reviews.

We have a multidisciplinary team approach to patient care with GP's having protected time during clinics to support other members of the clinical team.

We have a frailty team made up of GPs who specialise in frailty, mental health nurses, care coordinators and social prescriber link workers.

We are a forward-thinking practice embracing a team-based model for delivery of patient care and the use of new technologies to support this.

Main duties of the job

The ANP will act as an autonomous practitioner working independently and in conjunction with other health care professionals to access, diagnose and treat the conditions of patients attending the GP practice; to demonstrate clinical thinking in the clinical decision making process, initiating direct referrals as appropriate.

They will carry out 2-3 days carrying out clinics for urgent and routine appointment reviews and home visits and 1-2 days working within the frailty team carrying out care home reviews, home visits and frailty reviews.

Working in collaboration with the general practice team to meet the needs of patients; supporting the delivery of policies and procedures, providing leadership as required and promoting self-care in order that patients feel empowered to manage their own health care needs.

At all times, working within professional guidelines and codes of conduct; and within all relevant policies and procedural guidelines; to include infection control, chaperoning and risk management.

About us

Our clinical team is made up of GPs, ANP's, Mental Health nurses, Physician Associates, Pharmacists and Pharmacy technicians and our nursing team. They are supported by a team of receptionists, care coordinators, prescribing clerks and medical secretaries. We offer 8 hour shifts or longer days depending on the individual. We are supportive of training and career progression.

Guiseley and Yeadon Medical Practice will be operating a Tier-Based Delivery of Care, with their patients being segmented into 3 Tiers 1, 2 and 3.

Job description

Job responsibilities

ANPs will be expected to help deliver care to each of these tiers on a rota basis.

- Tier 1 represents those patients with the lowest physiological risk factors for illness and no other significant or combinations of chronic health problems. The ANP will be part of a multidisciplinary team compromised of GPs, Nurse Practitioners, Advanced Clinical Practitioners and Physician Associates. They will have their own appointment schedule working within the wider MDT team and have an available Tier 1 GP to consult with over any more complex problems and queries.

- Tier 2 represents patients who have one or more significant chronic health problem or are at a time in their life of increased physiological risk for example, pregnancy or older age. This is the group of patients where continuity of care can have the greatest impact on patient outcomes and the practice will be striving to achieve this. The ANP here will work alongside GPs and the nursing team delivering chronic health management, with their own appointment schedule but also supporting the nursing team where appropriate. The ANP will be involved in liaising with PCN health professionals, community and secondary health regarding patient care.

- Tier 3 represents the most complex patients, these patients maybe palliative, represent safeguarding risks or be going through a time of crisis which is impacting on their health. Care home patients will be included in this Tier and the ANP will be expected to support the MDT teams and meetings to plan these patients care. These patients are likely to require the ANP at times to perform home visits where remote consults are not appropriate.

- Administrative duties such as issuing of prescriptions, checking pathology results, tasks and incoming letters/documents for the practice will be performed in dedicated working time in the appointment schedule or a dedicated session as part of the rota.

- The ANP with appropriate training may also be asked to participate and hold Group Consultations.

- The ANP will also be expected to attend the daily team huddles.

Assess diagnose and treat patients attending with a range of acute, non-acute and chronic medical conditions and refer to appropriate agency.

Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs or with undifferentiated diagnosis

Work within the Tier-Based Care Model appointment schedule with different sessions delivering care to Tiers from 1-3.

Take a share of home visits

Undertake face to face appointments, telephone and video consultations

Review and action path reports

Directly admit patients to secondary care hospital in acute medical need

Clinically examine and assess patients needs from a holistic approach including the physiological and psychological perspective, and plan clinical care accordingly.

Proactively identify diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate

Undertake and interpret a range of diagnostic tests and routine clinical procedures e.g. cytology, venepuncture and blood pressure monitoring.

Undertake and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and local formularies

Work with patients and their carers in order to support compliance with and adherence to prescribed treatments

Provide information and advice on prescribed or over the counter medication or medication regimens side effects and interactions

Prioritise health problems and intervene appropriately to assist the patients in complex, urgent or emergency situations, including initiation of effective emergency care

Read code all treatment and results to support the QoF and excellent patient care

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Develop care plans with the patients and their family and regularly monitors and updates

Assess, identify and refer patients presenting with mental health needs in accordance with guidelines

  • Educated to Degree level Qualified or working towards a MSc in relevant area
  • Independent Non-Medical Prescribing Qualification in Minor Illness/Minor Injuries

Experience

  • Evidence of sound post-registration professional practice Demonstrable experience of working in the specific field where the post is held
  • Evidence of continuing professional development (CPD) relevant to the clinical area to advanced level e.g. Degree/Masters/post-graduate diploma and equivalent knowledge /experiential learning
  • Evidence of advanced practice skills and able to demonstrate the impact of this on practice change/development
  • Evidence of promoting/supporting active user/care involvement/participation
