Job summary

Folkestone, Hythe and Rural Primary Care Network (FH&R)is an innovative, friendly and forward thinking group of practices. We are located in the historical South Kent Coast of the Garden of England (Kent). General Practice is changing at pace and we see this as a positive and opportunistic challenge.

We are recruiting to expand and develop our existing PCN Pharmacy Team. This is an exciting opportunity to join an existing team consisting of a lead senior clinical pharmacist, two clinical pharmacists, three pharmacy technicians and a care coordinator for medicines . We are hoping to recruit to expand our team. We are looking for enthusiastic pharmacists, who wish to develop a scope of practice and work as part of our team to make an impact on patient outcomes in primary care.

These posts are full time, but part time will be considered. Salary will be agreed depending on experience and competitive rates for applicants who have completed a CPPE primary care pathway course and have an IP qualification are available. We have supported pharmacists moving from other sectors to working in primary care and we believe strongly in investing in training and development.

Main duties of the job

The main duties involve working collaboratively as part of the clinical pharmacy team and wider multi-disciplinary team, towards improving the health and wellbeing of patients. To improve the effective use of medication by patients through the application of patient centred care and evidence-based medicine. To provide advice and support for both patients and colleagues. To assist practices in safe and cost-effective medicines management.

The PCN is looking for an individual with a positive attitude where anything is possible and know that you can be an effective member of our pharmacy team, engaging with the senior team and colleagues in order to effect planned changes. The PCN is committed to increasing the skills and competencies of staff, as such you will need to engage with development opportunities. You will be instrumental in implementing processes to ensure identified priorities are delivered, you will also support the clinical development of other team members.

Folkestone, Hythe & Rural PCN comprises 7 practices with a combined list size of c.53,000 patients. We are proud and enthusiastic to be an inventive front runner in the new era of The Primary Care Network.

We are a forward thinking, friendly and cohesive PCN. Our Clinical Directors and Senior Core Team are committed to the development of our PCN, to provide exemplary services to the patient population. We work closely with NHS England. One project is 'The Primary Care Network Access Innovation Pilot : the Folkestone Hythe and Rural Hub' (see NHS England website and video link), which has improved patient access to services and managed increased demand on our GP practices.

We are a rapidly expanding PCN team which invests in staff and their development. We are excited at the prospect of expanding the Pharmacy Team and the pharmacy services that can be introduced or further devleoped.

Job description

Job responsibilities

The following are the core responsibilities of the clinical pharmacy team and the clinical pharmacist will be required to undertake any of the responsibilities listed. Post holders with less experience will be supported to develop competencies to be able to undertake any of the responsibilities below. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a.To provide medication reviews with patients either within practices face to face or telephone or within their care home setting targeting high risk patients, patients with long term conditions or frailty.

b.To support and advise the PCN Pharmacy Team technicians and other PCN staff members with medication issues

c.Practice as an independent prescriber working within scope of practice, if qualified to do so.

d.Review and implement safe prescribing systems for vulnerable patients, improving compliance

e.Support practice teams in understanding wider system issues e.g. alerts, OTC, self-care and homely remedies.

f.Administer influenza and other vaccines, if qualified to do so.

g.To provide advice and support on medication monitoring and systems to ensure compliance by practices.

h.To attend multi-disciplinary team meetings to provide advice and guidance on the medication use and prescribing, receive referrals and raise concerns

i.Assist when required with clinical audits and working to achieve clinical targets.

j.Act as the practice point of contact for medicines related matters, establishing positive working relationships

k.Support clinicians with the management of patients suffering from prescription drug dependencies

l.Completion of the CPPE training pathway Primary care pharmacy education pathway, unless completed previously or completed equivalent course.

In addition to the primary responsibilities, the clinical pharmacist may be requested to:

a.Provide education on medicines for the clinical team

b.Assist with QOF and other prescribing targets achievement for practices.

c.Review the latest guidance ensuring the practice conforms to NICE, CQC etc.

d.Encourage cost-effective prescribing within the practice.

e.To reconcile medicines following patients discharge from hospital or clinic appointments.
