Main duties of the job

Responsibilities

  • Birth Month Recall system
  • Workforce Rota and Staffing Levels
  • Manage and update telephone system, practice website and social media channels.
  • Care Planning for patients - contacting patients with unplanned admissions to hospital to follow-up care.
  • Holiday Cover and Absence Management
  • Deal with escalated queries from patients & clinicians.
  • Liaise with Clinicians to identify changes and updates required.
  • Update, review and design - protocols, polices and standard operating procedures
  • HR & Line Management of the Care Co-ordinator workforce - approachable, engaging and proactive.
  • Supervising and overseeing the day to day operational delivery of the Care Co-ordination Team
  • Lead team meetings, attend Heads of Dept meetings and other practice meetings where required.
  • Work alongside Data & Admin, Operations, Finance, Nurse Managers as Heads of Department reporting directly to the Practice and Pharmacy Manager.

Summary

We are looking for an enthusiastic Care Coordinator Manager to join our friendly and caring team here at The Downland Practice, we are a vibrant, forward-thinking GP practice that places the care of both our patients and staff at the core of everything we do. Our close-knit team fosters a warm, friendly, and compassionate environment, creating a space where everyone feels valued. Serving a diverse rural community of over 11,000 patients, our dedicated GPs, nurses, pharmacist, healthcare assistants (HCAs) and administrative staff work together to provide outstanding care. Every day, our team goes above and beyond to support and improve the health and well-being of the community we are proud to serve.

Applicants should enjoy working in a vibrant fast paced environment and possess excellent communication & IT skills (Excel, OneNote, Word etc). There are several systems we use and so this is paramount. They must be able to provide excellent customer service to our patients and possess the ability to manage and prioritise a busy/varied workload. Common sense, flexibility, compassion and a "can do" approach is essential for this position.

  • Experience with care plans and senior care is an advantage.
  • Previous experience in a medical environment essential.
  • Customer Service experience with excellent people skills.
  • Be flexible as the role will also include additional holiday/sickness cover when required.
  • The post is subject to the completion of a 6 month probationary period & a satisfactory DBS Check.
  • Flexible approach to work.
  • Adaptable to change
  • Minimum of 5 GCSEs (or equivalent) at C grade or above, including
