Job summary

Altrincham Healthcare Alliance PCN is looking to recruit an experienced Physician Associate under the additional roles reimbursement scheme to work across its five practices. You will be accountable to the Primary Care Network, working in individual practices and supporting that practice team.

Main duties of the job

  • Provide primary support to general practice by assessing, managing and treating patients of all ages with a variety of acute, undifferentiated and chronic conditions.

  • Be confident, in not only dealing with patients face to face, but also in assessing and treating them via telephone/video consultations.

  • Be responsible for making referrals to specialist services, review and act on laboratory and Imaging results.

  • Carry out home visits or visit nursing and residential homes.

  • Offer specialised clinics, following appropriate training, including (but not exclusively) family planning, baby checks, COPD, asthma, and diabetes.

  • Make a significant contribution towards each practice meeting their clinical targets and objectives.

  • Work very closely with Multidisciplinary Teams and the Primary Care teams within each Practice.

Altrincham Healthcare Alliance Primary Care Network (PCN) is providing an exciting opportunity for a Physician Associate to join a proactive PCN of five GP Practices covering 48,000 patients.The ideal candidate will be a forward-thinking, enthusiastic individual with initiative who is keen to work within a team of like-minded people and deliver excellent care to our population in south Trafford.

You will become part of an evolving MDT with clinical support from the Lead GP. Working across the five GP practices within Altrincham Healthcare Alliance PCN, you will work closely with GPs, nurse practitioners, social prescribers and pharmacists as part of a multidisciplinary team while also building strong effective working relationships with the local community, hospitals and care home teams to improve the care and services to our patients.

The role will focus on PCN initiatives as set out in the PCN contract, NHS Long Term Plan and deliverables agreed by the network members to improve place based patient care

Job responsibilities

Participate in all treatment and preventative healthcare services in the practice as delegated and agreed by the supervising physician(s).

Provide direct clinical care to patients using established clinical guidelines, professional and practice policies.

Perform a holistic assessment to inform an appropriate management plan.

Examine, assess, diagnose and treat patients with a range of acute, non-acute, chronic and long term medical and mental health conditions in the surgery or their own place of residence (if requiring acute medical attention) in a timely manner.

Refer patients to other healthcare practitioners within the practice multi-disciplinary team or wider services/medical specialities as appropriate and in consultation with supervising clinician.

Perform routine diagnostic and therapeutic procedures *(excluding ionising radiation)

Request, interpret and communicate diagnostic and test results to patients and the multi-disciplinary team, and initiate the appropriate follow up actions as required.

Undertake patient counselling and health promotion to facilitate preventative care and self-management.

Monitor and follow up changes in patients condition and response to treatment.
