Job summary

Altrincham Healthcare Alliance Primary Care Network (PCN) is providing an exciting opportunity for a Clinical Pharmacist to join a proactive PCN of five GP Practices covering 48,000 patients.The ideal candidate will be a forward-thinking, enthusiastic individual with initiative who is keen to work within a team of like-minded people and deliver excellent pharmacy-led services to our population in south Trafford.

You will become part of an evolving team of Clinical Pharmacists with clinical support from the Lead GP. Working across the five GP practices within Altrincham Healthcare Alliance PCN, you will work closely with GPs, nurse practitioners, social prescribers and existing pharmacists as part of a multidisciplinary team while also building strong effective working relationships with the local community, hospitals, the PCN community pharmacy lead and wider pharmacy teams to improve the care and services to our patients.

The role will focus on PCN initiatives as set out in the PCN contract, NHS Long Term Plan and deliverables agreed by the network members to improve place based patient care.

Main duties of the job

  • Management of patients with chronic diseases to proactively manage patients with complexpolypharmacy
  • Clinically assess and treat patients using their expert knowledge of medicines for specificdisease areas
  • Provision of specialist expertise in the use of medicines whilst helping to address publichealth and social care needs and tackling inequalities across the PCN area
  • Development of relationships and close working with pharmacy professionals across thewider health and social care economy
  • Provision of leadership on person-centred medicines optimisation and quality improvement
  • Delivery of first-class, professional service to support patients and GP practices across thePCN

To support Altrincham Healthcare Alliance Primary Care Network in all aspects of medicines optimisation.

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to:

• Management of long-term conditions

• Minor illness

• Management of medicines on transfer of care

• Reviewing systems for safer prescribing

• Contributing to repeat prescription authorisations and reauthorisation.

• Action acute prescription requests

• Addressing both the public health and social care needs of patients

• Contributing to achievement of QOF and locally commissioned quality improvement schemes.

• Undertaking clinical audit

• Developing, promoting and implementing the high quality, evidence based and cost effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial).

Qualifications

Essential

  • Professional knowledge acquired through vocational masters degree in pharmacy (4 years) or equivalent + 1 year pre-registration training.
  • Holds or working towards an independent prescribing qualification
  • Successful completion of/working towards completion of the CPPE Clinical Pharmacists in General Practice Education Programme
  • Demonstrates and understanding of, and conforms to, relevant standards of practice.
  • Follows professional and organisational policies/procedures relating to performance management

Desirable

  • Postgraduate Diploma in Clinical Pharmacy

Experience

Essential

  • Commensurable experience working at foundation level of the RPS pharmacy framework, demonstrated within practice portfolio
  • Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice
  • Demonstrates ability to integrate general practice with community and hospital pharmacy teams and community groups
  • Experience of partnership/collaborative working and of building relationships across a variety of organisations
  • Post registration experience minimum of two years
  • Experience of working in the NHS
  • Experience of communicating and engaging effectively with internal and external agencies/ stakeholders
