Longton Health Centre are searching for an experienced and highly motivated Advanced Primary Care Practitioner to join our friendly team - Primary Care Practitioner Paramedic or Advance Nurse Practitioner.
We are looking for a motivated and positive individual who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice.
They will work under the GPs and collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
We are a balanced clinical team consisting of GP Partners, salaried GPs, Advance Nurse Practitioner and full Pharmacy team along with full Nurse Healthcare Team. Our clinical teams have excellent non clinical support from our leadership and management teams along with patient administrative support from our Administrative teams.
We are a pro-active member of Ribble Medical Group Primary Care Network as the largest network practice working to deliver transformational models of care via social prescribing and first contact physiotherapy
Main duties of the job
The role will involve seeing patients in practice with acute conditions, on the day online and face to face triage and co-morbidity reviews.
The role will require the Advanced Clinical Practitioner to be clinically confident to see a full range of patients presenting with undifferentiated diagnoses to assess, treat, prescribe, referring as necessary all those in both the practice environment and within our local community.
With an aim to deliver a high standard of patient
care as Advanced Clinical Practitioner in general practice, using
clinical skills, and a broad theoretical knowledge base
About us
Based within the semi-rural village of Longton we are a friendly, high performing, forward thinking practice, with a current list size standing around 10500 patients. We are proud to be a training practice and accommodate medical students from UCLAN and Manchester Deanery. We work within the Ribble Medical Group Primary Care Network comprising of 7 local practices that meet regularly and have a developed network Nursing forum which provides both networking and training opportunities.
The post is for up to 37.5 hours per week however consideration for part time or more up to full time applications with a review of hours that would suit the individual and practice need. Salary in line with qualifications and experience

- To manage own clinical workload in general practice responding
effectively to patient need and ensuring patient choice and ease of access
to services.
- To mentor and support other nurses in developing and maintaining
clinical skills
- To always work as an effective member of the Practice team to
ensure good quality care is afforded to all patients.
- Work under the leadership of the GPs and liaise with GPs where patients
needs are outside of ANPs capabilities
- Undertake assessments of patients, making use of skills in history
taking, physical examination, problem-solving and clinical
decision-making, to establish a diagnosis and management plan
- Instigate necessary diagnostic tests or investigations
- The post holder is an independent prescriber: to prescribe safe, effective,
and appropriate medication as defined by current legislative framework
- Provide safe, evidence-based, cost-effective, individualised
patient care
- Offer a holistic service to patients and their families, developing
where appropriate an on-going plan of care/support, with an emphasis on
prevention and self-care
- Promote health and well-being using health promotion, health
education, screening, and therapeutic communication skills
- Refer patients directly to other services/agencies as appropriate
- Work with nursing, medical and health care assistant colleagues to
ensure that National Service Frameworks (e.g., Coronary Heart
Disease/Older People/ Diabetes /Mental Health) are being delivered

the wider CCG to promote integrated and seamless pathways of care

existing staff, such as medical students, NP students, GP registrars, GP
Assistants within the Practice.

guidelines and the development of practice-based research.
- Monitor the effectiveness of their own clinical practice through
the quality assurance strategies such as the use of audit and peer review

aspects of the role is maintained
