Job summary

The Bridges Medical Practice is looking for an Advanced Nurse Practitioner. Ideally 10 sessions but less will be considered for the right candidate. We are a busy practice with over 16000 patients based across 2 sites in Weymouth. We currently have 6 Partners as well as Salaried GPs and Retainers. We support outside interests and portfolio careers. A generous Golden Hello will be offered to the successful candidate depending on number of sessions and experience.

Main duties of the job

We are looking for an ANP who is a team player, and committed to Primary Care . Currently we provide a mix of triage, routine phone calls and face to face appointments but are continually adapting to the changing situation. The successful candidate would do a mix of these as well as a share of home visits. There is the opportunity to lead in a specific area of interest.

    Job responsibilities

    Job Summary: The post holder is an experienced nurse who, acting within their professional boundaries, will provide care to presenting patients who wish to be seen by a health care professional. Working as a skilled nurse practitioner, the post holder will autonomously assess, diagnose and treat patients with potentially undifferentiated and undiagnosed conditions. The post holder will demonstrate safe clinical decision making and expert care. The Nurse Practitioner will triage and prioritise patient needs and initiate clinically effective treatment. Any necessary investigations and referrals onto secondary care will be carried out in a timely and appropriate manner. The Practitioner will manage a clinic session, with pre-booked appointments and visit patients in their own homes. Job Responsibilities: Work autonomously for the Bridges Medical Practice, Examining, Diagnosing and treating patients presenting with minor illness including appropriate prescribing • Undertake a full and comprehensive health history and examination to form a working diagnosis, using a person centred approach and to document this fully in the practice computerised notes system • Accurately diagnose, monitor and manage • Make an evaluation of the treatment the patient can be offered, discuss and agree this with the patient • Initiate laboratory requests and collect specimens as required. • Provide health education as appropriate, • Referral to other members of the primary and secondary health care team as necessary • Undertake chronic disease management integrating both drug- and non-drug-based treatment methods into a management plan • Treat minor conditions, this may include cryotherapy • Provide telephone support or advice to patients regarding their medical condition or ongoing treatment. • Provide comprehensive contraceptive management including advice on emergency contraception, full contraception options and sexual health advice. • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice • Where there is an indication that a child, elderly or vulnerable person may have been abused, the Practitioner should follow local protection guidelines and refer as appropriate or discuss with a senior member of the medical team • Carry out nursing procedures, referrals and treatments according to agreed Bridges Medical Practice and Dorset CCG policies and Procedures. • Be able to recognise limitations in practice as defined by the NMC Code of Practice. • Ensure full informed patient consent is obtained prior to any procedure undertaken. Administration and professional responsibilities • Participate in the administrative and professional responsibilities of the Practice team • Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes • Ensure the clinical computer system is kept up to date with accurate details recorded and amended • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice • Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit • Attend and participate in Practice meetings as required • Attend and support Nurse Practitioners within clinical supervision
