Job summary
We are looking for an enthusiastic, self- motivated and well
organised Clinical Pharmacist for an exciting opportunity to work within Mendip
Primary Care Network, consisting of 5 practices. We will consider full or time working.
The Mendip Primary Care Network are a forward thinking and
dynamic group of 5 practices with 37,400 patients, with a history of working
together, both within Mendip Federations and the Your Health and Wellbeing
Mendip (YH&WM) group of practices. We are seeking a like-minded practice
pharmacist, preferably with previous experience of working in primary care, to
develop and manage medicines optimisation services within our practices.
This is an exciting opportunity to work as part of the PCN
Pharmacy team, The newly formed Mendip Complex Care Team and to work alongside
each of the general practice team to improve value and outcomes from medicines.
and consult with and treat patients directly. This includes providing extra
help to manage long-term conditions, particularly patients recently discharged
from secondary care and advice for those on multiple medicines. The role is
pivotal to improving the quality of care and ensuring patient safety.
Having a clinical pharmacist in post means that GPs can
focus their skills where they are most needed, for example on diagnosing and
treating patients with more complex conditions. This helps GPs to manage the
demands on their time.
Main duties of the job
JOB SUMMARY:
The post
holder will work as part of a multi-disciplinary team in a patient facing
role. The post holder will take
responsibility for undertaking chronic disease management review (with patients
where appropriate) to proactively manage patients with complex polypharmacy.
The post
holder will provide clinical leadership on patient centred medicines
optimisation and quality improvement, whilst contributing to the quality and
outcomes framework and enhanced services.
The post
holder will be a prescriber or will be completing training to become a
prescriber and will work alongside their colleagues in the PCN Pharmacy team, the Complex Care Hub and the general practice teams.

About us
Mendip PCN consist of 5 practice: Mendip Country Practice acts as the host practice, together with Beckington Family Practice, Grove House Surgery, Oakhill Surgery and Park Medical Partnership.
The Pharmacy team is based at Highfield House which is part of the Mendip District Council complex in Shepton Mallet. The Complex Care Hub, Health Connections and Home visiting Paramedics are also based in the same building and you will work closely with each team. The atmosphere is friendly, relaxed and patient orientated.
At present the Pharmacy team consists of one Clinical Pharmacists, 2 Pharmacy Technicians and one Pharmacy Care Co-ordinator. Or aim is to exband the team to it's maximum potential.
There will be occasional visits to the above surgeries.

and limits of competence. Signposting to
practice dispensary, community pharmacy and referring to GPs or other
healthcare professionals as appropriate.
Management of repeat
prescribing reauthorisation process by reviewing requests for repeat
prescriptions and medicines reaching review dates.
Provide specialist
expertise in the use of medicines whilst addressing both the public health and
social care needs of patients in the Mendip PCN and help in tackling
inequalities.
To have a leadership
role in supporting further integration of general practice with the wider
health care teams (community and hospital pharmacy) to help improve patient
outcomes, ensure better access to healthcare and help manage general practice
workload. The role has the potential to
significantly improve the quality of care and safety for patients.
Develop relationships
and work closely with other pharmacy professionals across PCNs and the wider
health and social care system.
Take a central role
in the clinical aspects of shared care protocols, clinical research with
medicines, liaison with specialist pharmacists, community pharmacists and
anticoagulation.
