Job summary
The
Health Connector will work in Mendip PCN delivering person-centred care and
support as part of the Health Connections Mendip (HCM) team. Health Connectors
support people to better manage their health and wellbeing in the community through
a combination of individual and group work.
Social prescribing; helping people to
improve their health, wellbeing and social welfare by connecting them to
community services. By increasing peoples active involvement with their
local communities, personal and community resilience is strengthened and health
and wellbeing inequalities can be reduced.
Deliver HCM projects within the community,
including Talking Cafés, Talking Benches and other group health education
programmes.
Main duties of the job
Identify gaps in service provision
through speaking to patients and feed back to their line manager.
Provide one-to-one and group
health coaching support for people with one or more long-term health conditions,
based on what matters to them, with the aim of: improving peoples levels of
activation, empowering people to manage their own health and improve their
health outcomes.
Manage and prioritise a caseload
of patients. Understand when it is appropriate or necessary to refer people
back to, or on to, other health professionals/agencies.
Support the promotion of the Health Connections Mendip
directory/website, resources, training and one-to-one support across the
locality.
Attend and contribute to team,
practice and PCN meetings and events as required by the service.
Participate in regular supervision, reflective practice and continual
learning.

5 practices form (Central) Mendip PCN, these are Mendip Country Practice, Coleford, the host practice, Beckington Family Practice, Grove House Surgery, Shepton Mallet, Oakhill Surgery and Park Medical Practice, Shepton Mallet. We are a thriving forward thinking PCN, the Health Connections team is a long established team orginally created by the vision of Frome Medical Practice. You will work closely with practices and the other teams within the PCN, namely Complex Care Hub, Pharmacy Team and the home visitng Paramedics.
The teams work from Highfield House, in the Shape Mendip Complex and in Fromefield Surgery, located in Frome Medical Practice. The teams work extremely well together, and are much appreciated by all surgery staff and patients alike.

- 5 GCSEs at A-C or equivalent including English and Maths.
- Demonstrable commitment to professional and personal development.
- Willingness to undertake relevant in-house training.
Desirable
- Degree or other qualification in relevant area (or equivalent work experience)
Experience

- Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (in a paid or unpaid capacity).
- Experience running and facilitating groups.
