Job summary

Care Co-ordination in Central Mendip for care home residents of the PCN aligned care homes

The post holder will be responsible for the provision of wide-ranging and efficient support to Mendip Primary Care Network. They will work with the PCN to proactively identify and work with care home residents according the PCN DES specification for Enhanced Health in Care Homes and provide coordination and navigation of care across health and other care services.

The care coordinator will liaise with the care homes directly to establish a two-way relationship to represent residents at the weekly MDT meetings on behalf of the PCN.

Main duties of the job

Enhanced primary and community support

MDT support co-ordinating health and social care

Working with falls prevention teams (re-enablement and rehabilitation) and the care homes

Co-ordinating high quality palliative and end of life care, mental health and dementia care

Input into helping develop joined up commissioning and collaboration between health and social care.

Job responsibilities

The post holder will contribute to tackling inequalities in health and social care regarding individuals with long-term conditions within the care home setting. An ethos of promotion of independence and partnership-working is integral to this post.

As a patient facing role the post holder will also be responsible for a caseload of patients identified through the MDT meetings. Support provided directly with patients and their carers would include co-producing personalised plans, utilising decision aids, providing information and training opportunities, making appointments, coordination and navigation for people and their carers across health and care services.

A key part of the role of a care coordinator role is in the care Homes MDT: improving the continuity of care by acting as a point of contact for residents, families and professionals who visit care homes, such as MDT members and in-reach specialists.

They will work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed. This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.

They will support the MDT with the weekly virtual home round through identification of people in need of review, or collation of information on people requiring an MDT review in addition to providing coordination, secretarial and administrative support to the PCN into and from the MDT.

  • Vocational level 3 in Business Administration or relevant
