Job summary
The post holder
will provide primary care services to patients who are homeless, many of whom
are experiencing addiction, mental health and physcial complex health problems
often with co-morbidities. The post holder will work in a GP clinical Lead,
multidisciplinary team of mental health nurses, including outreach nurses,
psychotherapist, primary care nurses, pharmacist and administrative support
staff to support patients who often have chaotic lifestyle choices, drug and
alcohol issues.
Main duties of the job
In
accordance with the practice timetable, as agreed, the post-holder will make
him/her-self available to undertake a variety of duties including surgery
consultations and emergencies, telephone consultations and queries, triaging,
visiting patients at home, checking and signing repeat prescriptions and
dealing with queries, patient test results, patient, referral letters
NHS/private, paperwork and correspondence in a timely manner;
Making
professional, autonomous decisions in relation to presenting problems, whether
self-referred or referred from other health care workers within the
organisation;
Recording
clear and contemporaneous IT based consultation notes to agreed standards;
Collecting
data for audit purposes;
Compiling
and issuing computer-generated acute and repeat prescriptions (avoiding
hand-written prescriptions whenever possible);
Prescribing
in accordance with locally agreed or national guidelines.
In
general the post-holder will be expected to undertake all the normal duties and
responsibilities associated with a GP working within primary care.
About us
An exciting opportunity has arisen for a 6 session GP to join
a hardworking, compassionate multidisciplinary team working at Birmingham
Health Exchange. The role is to support vulnerable groups of people who face barriers
to accessing care including primary care services. The post holder will be
working with patients with past and current substance misuse issues,
experiencing homelessness or are insecurely housed, vulnerable and isolated.
Patients who struggle to engage with care for a multitude of reasons and have
complex needs are being support by a team including Mental Health Nurses, Outreach
Nurses, Psychotherapist, plus primary care GPs Nurses and administration
staff.We have an holistic approach to healthcare and our work is
informed by social detriments of the local and national health needs. We pride
ourselves in giving a responsive and person led approach finding compassionate
solutions for patients who need support accessing all care systems.

Essential
- - A vocationally trained and accredited GP.
- - Excellent communication and interpersonal skills.
- Ability to work as part of a multi-disciplinary team.
- - Understanding of the pressures faced by GPs and healthcare teams.
- - Willingness to contribute to and participate in a peer support group.
- - Current registration with the GMC and has membership with a recognised medical defence organisation;
- - Strong levels of IT proficiency and familiarity with primary care clinical systems (the Practice uses EMIS Web). Some experience in using the Microsoft suite of programmes;
- - Proven ability to offer support within a clinical team and share learning points in Practice meetings;
