Job summary

To work within the Multidisciplinary Team (MDT), to provide North Trafford PCN with a social prescribing service that takes a holistic approach, has a focus on what matters to me and connects patients to community based services and support to enable them to maintain their health and wellbeing, support behaviour change to achieve a healthy lifestyle, and which addresses health inequalities

Main duties of the job

1. To work with and support individuals to identify their needs, develop personalised care and support plans, build up their resilience, and increase their active involvement with their local communities.

2. To work with and support community groups to be accessible and sustainable and help people start new community groups, working collaboratively with all local partners.

About us

We are innovative and forward thinking Primary Care Network. We have 4 member practices in Stretford and Old Trafford. Our population consists of people from various ethnic backgrounds. Also we have health inequalities and deprivation in our catchment area.

Job description

Job responsibilities

1. Take referrals from a range of staff (clinical and non-clinical) within the Multidisciplinary Team.

2. Provide personalised support to individuals, their family and carers, with a focus on what matters to me, to enable them to take control of their health and wellbeing, live independently and improve their health.

3.Take a holistic approach, based on the persons priorities and the wider determinants of health.

4. Work with individuals to co-produce a simple personalised care and support plan that will focus on improving health and wellbeing, introducing or reconnecting them to appropriate community groups and statutory services.

5. Manage and prioritise a caseload, in accordance with the needs, priorities and any urgent support required by patients who are referred to the service.

6. Where appropriate, refer the patient back to health professionals/agencies, e.g. if the persons needs are beyond the scope of social prescribing.

7. Work collaboratively with local partners and communities to identify local community assets (Voluntary, Community and Social Enterprise (VCSE) organisations), and support them to receive social prescribing referrals, ensure they are open and welcoming, can provide opportunities for people to develop friendships and a sense of belonging, build new skills and confidence.

8. Support local groups to become sustainable, increase their capacity and and have basic safeguarding processes for vulnerable individuals.

9. Identify gaps in local community provision and contribute to developing new groups and networks to fill gaps.

Experience

Essential

  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work).
  • Experience of supporting people, their families and carers in a related role (including unpaid work).
  • Experience of working with the VCSE sector (in a paid or unpaid capacity), including with volunteers and small community groups.
  • Experiences of partnership/collaborative working and of building relationships across a variety of organisations.
  • Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities.
  • Knowledge of community development approaches.
  • Knowledge of motivational coaching and interview skills.
  • Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports.

Desirable

  • Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity.
  • Experience of data collection and providing monitoring information to assess the impact of services.
  • Knowledge of the personalised care approach
  • Knowledge of VCSE and community services in the locality.

Qualifications

  • NCQ Level 3, Advanced level or equivalent qualifications, or working towards.
  • Demonstrable commitment to professional and personal development.

Desirable

  • Training in motivational coaching and interviewing, or equivalent experience.
