Job summary

Main duties of the job

To analyse and extract diagnostic and procedural information from patient notes and information systems and assign the appropriate code in an accurate and timely fashion.

Job description

Job responsibilities

Primary Duties

The following are the core responsibilities of the clinical coder. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

a. Accurately clinically coding data on EMIS into the patients medical record

b. Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action

c. Filing and storing records as required

d. Actioning all incoming correspondence via mail in a timely manner

e. Scanning patient related documentation and attaching scanned documents to patients healthcare records

f. Inputting data into patients' healthcare records as necessary

g. Answering incoming telephones calls, transferring calls or dealing with the caller's request appropriately

h. Carrying out system searches as requested

i. Maintaining a clean, tidy, effective working area at all times

j.Supporting all clinical staff with general administrative tasks as requested

Secondary Duties

In addition to the primary responsibilities, the clinical coder may be requested to:

a. Review and update the practice coding policy as required in conjunction with the Deputy Practice Manager

b. Partake in audit as directed by the audit lead

c. Support reception staff, providing administrative cover, as required

d. Book appointments

e. Complete opening and closing procedures in accordance with the duty rota

f. Action GP2GP and complete summerising duties
