Job summary

Provisional interview date: Wednesday 1st March or Wednesday 8th March 2023 TBC.

To act as the single point of administrative and coordination support for the Older Persons Integrated Care (OPIC) Team within the Barnstaple Alliance Primary Care Network.

This post is for fixed term maternity cover to start as soon as possible through to April 2024. The hours available are between 20 and 37 hours a week and we ask applicants to specify how many hours they would like to work a week within their application.

Due to rate of growth and development of our PCN, there may be an opportunity for the successful applicant to apply for a permanent position within our PCN at the end of the fixed term maternity cover contract. Main duties, responsibilities and hours of any future permanent roles may differ, and the position may be within a different area/team in the PCN but will still be an administration/coordinator role.

Main duties of the job

The OPIC Team consists of the Care Home Team and a Paramedic Visiting Team for housebound patients. The care co-ordinator role will be pivotal in ensuring all patients receive the best possible care and service through coordinating and supporting the delivery of all key activities of the OPIC Team. This includes access to services, advice and information, ensuring a patient centred approach with timely and effective care planning and organisation. The role will involve co-ordination of home visits for housebound and care home patients. The role will also include supporting digital initiatives and coordinating focused work around the delivery of the Network Contract DES specifications to include Enhanced Health in Care Homes (EHCH) and Structured Medication Reviews (SMRs).

Between us we provide care to over 52,000 patients in the Barnstaple area. Whilst retaining our independent surgeries, we are working together on collaborative projects to improve and develop the healthcare services we offer to patients.

MAIN DUTIES & RESPONSIBILITIES

Key responsibilities for the position will include but are not limited to the following:-

  • Provide efficient and comprehensive administrative and clerical support to the Care Home Team on a day-to-day basis.
  • Coordinate and arrange Long Term Condition reviews and vaccinations for both Care Home and Housebound patients for the OPIC team.
  • Continuously monitor, manage, and prioritise all incoming and outgoing correspondence and workflow for the OPIC Team (such as the OPIC teams email inbox and TPP SystmOne task boxes), initiating responses where appropriate.
  • Being the single point of contact for the OPIC team for the member practices and teams within the Barnstaple Alliance PCN, Care Homes and other healthcare professionals.
  • To work with the OPIC Team to identify and support the organisation of patient caseloads with Care Homes and the Cared for at Home.
  • Support the OPIC Team with inputting data and maintaining patient records and patient lists.
  • Practical application of TPP SystmOne and Microsoft Teams for coordinating workload, organising individual team members caseloads and appointments, and developing reporting mechanisms.
  • Coordinate agendas, prepare patient lists and take minutes of key meetings including the Multidisciplinary Team meeting.
  • Produce via TPP SystmOne and other digital tools reports to support the work of the team.
  • Support the PCN, its member practices and the Clinical Lead of the OPIC Team with aspects of the Network Contract Directed Enhanced Service (DES). Specifically, the Impact and Investment Fund (IIF), including the Enhanced Health in Care Homes (EHCH) and Structured Medication Review (SMR) areas.
  • Coordinate Care Home Ward Rounds, liaising with staff at all relevant organisations and booking patients into the Weekly Care Home Round appointment template within TPP SystmOne as needed.
  • To help Care Home Managers and GP Practices address their patients needs through answering queries, managing diaries and appointments, and delivering care plans.
  • To support the Home Care Team in the development of robust care plans in line with best practice.
  • Organising which elements of clinical (and QoF) checks need applying on visits.
  • Coordinate both acute and chronic visits for the Home Care team.
  • Order medical and pathology supplies as needed for the OPIC team.
  • Work with the PCN Operational Manager to plan and coordinate the OPIC Teams activity including holidays and sickness.

COLLABORATIVE WORKING RELATIONSHIPS

  • Recognises the roles of other colleagues within the organisation and their role in patient care.
  • Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. CCGs).

  • Liaises with other GP Practices and staff as needed for the collective benefit of patients.
