Job summary
The Barnstaple Alliance Primary Care Network (PCN) are recruiting a new and innovative Digital and Transformation Lead who will work closely with the PCN Management Team to support the transformation and the sustainability of general practice and the care services the PCN provide by developing digital solutions and capacity whilst improving accessibility.
Hours: 25 hours spread over the week to meet the needs of
the PCN to be agreed with the successful applicant
Salary: £31,746 - £38,480 FTE depending on experience
and qualifications
Main duties of the job
The Digital & Transformation Lead role aims to support
transformation and digital initiatives of the Barnstaple Alliance PCN to continue
to support good access for patient care and the overall sustainability of general
practice and PCN wide services.
The position will also play
a pivotal role in helping to identify and tackle health inequalities and improve
the quality of care and care outcomes for the most vulnerable of the patient
population. The role will use Population
Health Management as a tool to understand future demand, those most vulnerable to
that demand and proactively seeking solutions.

supportive network of four General Practitioner surgeries: Brannam, Fremington,
Litchdon and Queens Medical Centres. Between us we provide care to over 52,000
patients in the Barnstaple area. Whilst
retaining our independent surgeries, we are working together on collaborative
projects to improve and develop the healthcare services we offer to patients.

Digital and Transformation
- Work with the Management Team to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, expertise, and leadership to deliver changes in a collaborative way.
- Working with the Management Team, the post holder will help to develop strategy related to transformation and improvement, that make a difference to patients and staff in the PCN.
- Provide hands-on capacity and support to practices in implementing and sustaining agreed changes.
- Directly support delivery of transformation activity in general practice within the PCN with a view to improving patient access whilst protecting our workforce, quality and continuity of care, staff and patient satisfaction, and the efficiency and sustainability of general practice services.
- Support practices and staff to optimise use of existing digital functionality and new technology.
- Keep the Management Team and member practices updated on progress of current initiatives, capacity for change, progress, and issues so that plans can be adapted in response.
- Assess and review the PCNs transformation maturity, capacity, and local support needs.
- Comprehend and work alongside relevant frameworks to support and implement positive change where needed.
- Develop relationships with peers across other PCNs to share learning and expertise at place or system level.
- To work with the PCN Operational Manager to manage and ensure the PCN complies with all Data Protection Regulations/GDPR when sharing data which may include the production and implementation of DPIAs, Data Sharing Agreements, Assessments etc.
- Lead or participate in relevant working groups, project groups, service meetings and other forums across the PCN to agree changes, share learning, discuss challenges, and resolves issues.
- Drive integrated working, efficiency and reduce duplication or variation between practices.
- Disseminate new information across the member practices and be tasked with coordinating shared learning events/joint education initiatives.
- The post holder will work with the ICS to request specialist IT and estates expertise required to deliver transformation plans, in line with the ICSs delegated responsibilities for GPIT and GP estates.
DATA, POPULATION HEALTH MANAGEMENT AND HEALTH INEQUALITIES
- Identify digital solutions to support the development, resilience, and the futureproofing of the PCN in line with local, regional, and national needs.
- Improve adoption of new technology to enhance patient access and experience and increase PCN productivity.
- To review and improve the PCNs digital maturity.
- Take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made.
- Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges.
- Interpret data and highlight variation to support open discussion about where variation is and is not warranted.
- Help PCN and practice staff understand the limitations of particular data sets and where other information may be available to supplement understanding of issues.
- Have a good understanding of the broader drivers that contribute to health inequalities within the PCNs patient population and use this knowledge, along with the use of data management, to support the improvement of health outcomes.
