We are looking for a Full Time Wellbeing Team Leader to manage a growing team of Social Prescribers and Community Connectors to work across The Barnstaple Alliance. You will be joining the current team which consists of a Social Prescriber and a newly recruited Community Connector with exciting future involvement to expand the team further.

Main duties of the job

The overall purpose will be to coordinate the smooth day to day running of the Barnstaple Alliance Wellbeing Service. In this lead role you will be required to work closely, manage and coach the members of your team. Additional responsibilities will be to triage and monitor incoming referrals, lead the weekly allocation meeting and evaluate the progress of clients that have been enrolled into the Wellbeing Team.

You will also have Social Prescribing and flow co-ordination duties, managing an active caseload of more complex clients. You will need to engage with other external agencies to build rapport and form positive working relationships that will benefit both the Wellbeing Team and the clients.

About us

The Barnstaple Alliance PCN is a proactive and supportive network of four General Practitioner surgeries: Brannam, Fremington, Litchdon and Queens Medical Centres. Between us we provide care to 50,700 patients in the Barnstaple area. Whilst retaining our independent surgeries, we are working together on collaborative projects to improve and develop the healthcare services we offer to patients.

MAIN DUTIES & RESPONSIBILITIES

Management and Leadership of the Wellbeing Service

  • Take a leadership role in supporting and managing the Wellbeing Team.
  • Lead on network implementation of agreed developments and pathways with the Wellbeing Service, working closely with member practices and organisations.
  • To be a first point of management contact for the Wellbeing Service.
  • To be based at North Devon Voluntary Services (NDVS) and work flexibly across all Primary Care Network sites as required.
  • Developing relationships and working closely with other network Wellbeing Teams.
  • Overseeing the day-to-day operations of the Wellbeing Service and ensuring that any staff within the Wellbeing Team will achieve their primary responsibilities.

Service Management

  • Triage incoming referrals and allocate out to the appropriate team member via the weekly allocation meeting.
  • Ensure the Wellbeing Team maintains compliance with its NHS/CQC and any other contractual obligations.
  • In conjunction with the Practice Managers be the first contact for the Wellbeing Service enquiries and complaints from patients, dealing effectively and efficiently to promote public and staff satisfaction.
  • Lead on planning, evaluation and data analysis of systems and procedures to ensure adequate provision of the Wellbeing service.
  • Attend meetings as required on behalf of the Network, feeding back to Barnstaple Alliance PCN and members. This will sometimes be outside the normal working hours. (Time back in lieu to be taken).
  • Monitor and evaluate the progress of patients on the Wellbeing Teams case load.
  • Carry out any other duties as required, which are consistent with the duties and responsibilities of the post including Social Prescribing.

HR

  • In liaison with the PCN Lead, support and monitor the recruitment and retention of staffing levels and skill mix to meet the commitment of the Wellbeing Team.
  • Maintaining an effective overview of HR legislation.
  • To ensure induction, appraisal, and training are in place to enable staff to carry out their individual roles and responsibilities effectively.
  • Maintaining an overview of the Wellbeing Teams welfare.

Social Prescribing/Flow Co-ordination

  • Enrol identified people referred into the service. Gain consent, conduct guided conversations, providing advice and guidance to the client.
  • Identify vocational aspirations and negotiate a realistic route, ensuring that all clients have an agreed action plan to achieve these goals.
  • Ensure effective engagement with individuals to build confidence and resilience to manage their own needs without creating dependency.
  • Use the Flow Co-ordination model to manage clients with complex needs.
  • Assist a lead professional to organise 'Team around the Person' meetings and co-ordinate achieving patients goals to help them live more the life they want.
  • Liaise with a range of local clinical and non-clinical services to support the clients health and wellbeing.
  • Monitor and evaluate the progress of clients on your case load.
  • Develop strong working relationships, engaging with the Network and other organisations, in particular other social prescribing initiatives within the local area.
  • Develop an understanding of nature based and outdoor activities in the context of social prescribing and build knowledge of these areas North Devon wide.
  • Updating IT systems in a timely manner to ensure all data is complete and accurate.
