Job summary

We are looking to cover two roles with either one or two people. We would like 12 hours for contraception and women's health and 16 hours for diabetes and hypertension long term conditions.

Experience in these specialist areas would be an advantage but training could be provided for the right candidate.

Our normal working days are 8.30am - 6.00pm.  Some late night cover may be required on Wednesdays and Thursdays.

Salary scale between £16.00 - £22.00 based on qualifications, skills and experience.

Main duties of the job

To be responsible for the delivery of the Practice's Nursing Services working as part of a multi-disciplinary team ensuring provision of high standard of patient care.

To work closely with all clinicians and admin staff to ensure accuracy and safety of procedures and working practice.

Job description

Job responsibilities

Manage patient care in the Urgent Care Team (UCT) working alongside a multi-disciplinary team providing on the day clinical care to patients.

Provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.

Prescribe and review medication for therapeutic effectiveness appropriate to patient needs and in accordance with evidence-based practice.

Where necessary, evaluate patient test results in conjunction with the patient and their GP and ensure the appropriate course of treatment is initiated.

Support the nursing team and GPs in undertaking health promotion to help educate patients.

Provide both regular and emergency contraceptive advice. Carry out Pill checks, implants and coil check.

Assessing and prescribing in support of good wound management in complex situations (e.g. use of Doppler technique)

Carry out care to the patients in the treatment room, inc. but not exclusively smears, ECGs, ear syringing, dressings, etc.

Provide comprehensive travel health advice for patients prior to travel including malaria prophylaxis, safe sex, sun protection, food hygiene and vaccinations.

Administer child and adult immunisations and vaccinations under PGDs with relevant training in accordance with national and local programs. Competent in anaphylaxis and BLS or ALS. Ensuring safe storage, rotation and disposals of vaccines and drugs and that adequate stock levels are maintained.

Manage patients with chronic Respiratory disease, running specific clinic sessions and maintaining robust call and recall systems with the support of the admin team. Diagnose, monitor and develop individual management plans.

Liaise with appropriate colleagues in secondary care and other organisations as part of providing a smooth clinical pathway for the patient.

Demonstrate a sound knowledge of QOF and Enhanced services.

Enable the nursing team to apply and improve clinical governance and risk management systems.

Maintain accurate patient records using agreed templates and Read Codes.

Ensure accurate completion of all necessary documentation associated with patient health care.

Collect data and participate in regular Practice audit and research projects.

Contribute to the preparation of any Practice development plans.

Provide specialist clinical skills where trained to do so.

Mentoring/supporting students.

Maintain own annual registration with the NMC and adhere to Code of Professional Conduct.

Maintain a safe and therapeutic environment for all staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety) are implemented and adhered to, overseeing Infection Control policies and procedures.

Maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice.

Attend and participate in staff and Practice meetings and in-house clinical training sessions.

Any other reasonable duties required from time-to-time.
