Job summary

We are seeking to appoint a Care Coordinator at Partington Central Surgery. The post holder will be required to work with vulnerable patients from our practice location, both remotely and in person, and this is a brand-new service so will require a degree of flexibility and a range of duties that may vary as the service develops. In order to be considered for this role, you must be enrolled in, undertaking, or qualified from appropriate training as set out by the Personal Care Institute. If you are successful in this role, you will be working closely with our administrative teams, Social Prescribers, registered care homes, and clinicians to support the wellbeing and care needs of our patient population, including our registered care home population. The Care Coordinator will have a first point of contact for general advice and support and will also have clinical team oversight or supervision from our Lead Nurse where necessary

Main duties of the job

Key aspects of this role will include supporting the following GP Practice activities:

  • Direct Enhanced Services (DES)
  • Population Health Management
  • Impact and Investment Fund (IIF) and QOF (Quality and Outcomes Framework)
  • Co-ordinating the care of patients to most suitable services

About us

Partington Central Surgery is a single handed practice with three GPs, Practice Nurse, and Prescribing Clinical Pharmacists into the clinical/clinical support team. We have 5 receptionists with a view to recruit. We have a population list of around 4000.

Job description

Job responsibilities

Please note that the role of a Care Coordinator is NOT a clinical role. The post holder will have the following key responsibilities:

1. You will provide wide-ranging, proactive, and efficient administrative support to Partington Central Surgery;

2. You will be responsible for the administration, coordination, and monitoring of dedicated or targeted clinical campaigns;

3. You will develop close relationships with our CQC-registered care homes, and make sure that patients have correct identified and coded care support needs, in line with PCSP best practice;

4. You will work closely with our Data Quality and coding teams to assist the practices in meeting their contractual targets;

5. You will utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care, including through recalls for specific patient groups (e.g. immunisations, health checks, reviews, etc);

6. You will discuss patient-related concerns with the relevant staff and be supported to follow appropriate safeguarding procedures;

7. You will holistically bring together all of a persons identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person;

8. You will help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them - 2 - make choices about their care, including signposting people to take up training and employment, and to access appropriate benefits where eligible;

9. You will provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals;

10. You will support the coordination and delivery of MDTs within Partington Central Surgery;

11. You will work with the GPs and other primary care colleagues to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals; and

12. You will raise awareness of shared-decision making and decision support tools with both patients and clinicians. This is an outline of the responsibilities and outcomes required of the role.
