Busy and friendly General Practice looking for a highly experienced ANP, who has advanced clinical reasoning skills as well as being a confident decision maker.
We are looking for someone who is enthusiastic, dynamic, adaptable, and willing to work between 25-33 hours a week over 5 days. Part-time with a permanent contract on offer.

Main duties of the job
We are looking for a RGN registered ANP, who can independently prescribe. Chronic disease management in a Primary Care setting is essential.
Providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. You must demonstrate safe clinical decision making and expert care for patients within general practice. You will work collaboratively with the multi-disciplinary general practice team to meet the need of patients, supporting the delivery of policy and procedures.

To perform as an autonomous practitioner working independently and with other healthcare professionals to assess, diagnose and treat patients with a range of acute and chronic medical conditions, initiating direct referrals as appropriate.
Main Duties and Responsibilities:
General Duties:
- To provide ANP services to patients adhering to Practice Protocols
- To help in the development of new services that the practice wishes to implement
- Maintain excellent attendance, punctuality, personal appearance and hygiene
- Establish and maintain effective working relationships with co-workers and the general public
- To ensure that patients fully understand any procedure to be carried out on them, and the appropriate consent is obtained and documented.
- Support the GPs with chaperoning duties as required
- To ensure that principles of infection control are adhered to
Clinical Responsibilities:
- Work autonomously and be accountable for his/ her professional actions.
- To undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to registered patients.
- To take history, conduct appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow-up.
- Where appropriate make referrals to other healthcare professionals in primary and secondary care.
- Arrange hospital admission for appropriate cases.
- Undertake telephone triage work and provide telephone advice calls as appropriate.
- Prescribe medications as appropriate.
- Maintain accurate record keeping in Emis Web.
- Undertake regular medication reviews and medicines management.
- Request and interpret tests appropriate to patients needs.

- Dietary, exercise and lifestyle advice
- Blood pressure checks
- Urinalysis
- Health screening
- Routine injections, such as influenza
- Contraception and sexual health screening
Pathological specimens and investigatory procedures:-
- Perform investigatory procedures requested by the GPs
Chronic Disease Management:
- Be responsible for advising patients regarding Asthma and offering appropriate checks
- Be responsible for advising patients regarding diabetes and offering appropriate checks
- Be responsible for advising patients regarding COPD and offering appropriate checks
- Discuss and demonstrate the various methods of treatment. Teach the administration of the chosen method and ensure understanding of that method.
- See patients for follow-up
- Provide patients appropriate advice regarding steps needed to be taken to prevent worsening of their condition.
- To ensure full understanding of the Quality Outcome Framework (QOF) for chronic disease areas and to liaise with administration staff to ensure targets are achieved.
Administration and professional responsibilities:
- Participate in the administrative and professional responsibilities of the practice team
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
- Ensure the clinical computer system is kept up to date, with accurate details recorded and amended, using templates where applicable
- Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator
- Attend and participate in practice meetings as required
- Check NHS mail on a daily basis and respond to e-mails as required

- To participate in annual performance appraisal and review
- To take responsibility for ones own developmental learning including participation in supervision
- To take responsibility for maintaining a record of ones own learning and training
- To complete all training necessary for the role and any other training deemed suitable
- Attend and complete all mandatory training courses as appropriate to the post
