Main duties of the job
Supported by experienced
nurses within the practice, they will deliver care within the boundaries of
their role, focusing on supporting patients to be healthy, monitoring of long
term conditions, health prevention and screening activities. They will work
collaboratively to meet the needs of patients, ensuring the highest standards
of care and supporting the delivery of policy and procedures. The post-holder
will assist in the provision of patient care at all Haxby Group sites or
establishments.
The post-holder must work at
all times within their own competence and professional code of conduct as
regulated by the Nursing and Midwifery Council (NMC).
It is expected that the
post-holder will fully utilise all opportunities for learning under the
guidance of the nursing and practice leadership team and will demonstrate
evidence of professional development and increasing responsibility for patient
care and related duties.

Clinical Responsibilities:
- Assess, plan, implement and evaluate programmes of care to improve health and wellbeing and meet the healthcare needs of individuals and groups
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Enable patients, families and carers to develop their knowledge and understanding about their health and well-being and be as self-managing as possible
- Offer health promotion programmes by providing a holistic assessment, which may include:
- BP checks, height, weight, BMI, urinalysis and smoking status
- Breast health awareness - Cervical screening - New patient checks
- Well man/woman checks
- Advice for healthy eating, exercise, alcohol consumption and smoking
- Perform routine nursing tasks such as
- Venepuncture
- Recording ECG
- Spirometry
- Chaperoning and assisting patients
- Provide a holistic approach to wound management and implement wound care in line with current evidence-based guidelines
- Promote and deliver evidence-based care for patients presenting with aural conditions
- Promote and assist in the provision of evidence-based care for patients presenting with minor illnesses within own competence
- Offer advice about childhood and adult vaccinations and administer vaccines according to current guidelines and legislation
