We are looking for a Practice Nurse for 20 hours per week on a fixed term two year contract.

Main duties of the job
The post holder will work collaboratively with the multi-disciplinary team to meet the needs of the practice and patients, and will play an integral part in delivering the KPIs for the Practice.
You will have a varied caseload, including: Chronic Disease Management, preventative care, immunisations, patient education.

The Practice Nurse will work collaboratively with the General Practice team to provide and develop high quality, evidence based nursing services to the practice population. This role involves managing long-term conditions, delivering health promotion, disease prevention and supporting patients in self-management.

Key Responsibilities
Clinical Care
- Deliver General Practice Nursing services including chronic disease management (e.g. Diabetes, Asthma, COPD, Hypertension)
- Undertake cervical cytology, immunisations and vaccinations (child and adult), ECGs and health screening
- Provide health education and lifestyle advice to promote well-being and disease prevention
- Support patients in understanding and managing their long-term conditions through evidence based care and self-management plans
- Participate in infection prevention and control, vaccine storage and cold chain maintenance
- Perform clinical audits, maintain accurate records and ensure compliance with CQC, Infection Control and Safeguarding standards
- Participate in Quality and Outcomes Framework (QOF) and Enhanced Services Delivery
Leadership and Teamwork
- Work collaboratively with GPs, HCAs and the wider multidisciplinary team
- Support the development of nursing services within the practice and contribute to team meetings
- Supervise and mentor junior staff (e.g. phlebotomist, GPA, HCA) when appropriate
