Job summary

We are looking for a qualified Diabtes Nurse Specialist to join our team to provide clinical expertise and professional resource to our multi-profession team, patients and carers. The post holder will ensure the ongoing development and delivery of a caseload of patients, providing specialist advice within their clinical area for a cohort of patients with complex diabetic needs including history taking, clinical assessments, treatments, and education. They will also support our generalist practice nurses in development their diabetes care in the practice. You will provide excellent patient care and promote self-care management for patients diagnosed with diabetes.

The post hold must demonstrate critical application to detailed knowledge in the field of diabetes alongside management and leadership responsibilities. Highly organised and have excellent gold standards in quality and patient care.

Main duties of the job

To work autonomously in the provision of high quality specialist, evidence based diabetes care for people with diabetes within Mowbray Group Surgeries.

To promote self-care management to patients to prevent worsening of their condition and improve health outcomes.

To act as a specialist resource and provide specialist advice in relation to diabetes, to patients their family/carers and health professionals.

Mowbray House Surgery is a large, progressive Medical Practice based in Northallerton with a full time Branch Surgery in Hutton Rudby.

Key Responsibilities:The following are the core responsibilities. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  • To work autonomously in the provision of high quality specialist, evidence based diabetes care for people with diabetes within Mowbray Group Surgery from diagnosis to long-term management
  • To promote self-care management to patients to prevent worsening of their condition and improve health outcomes.
  • To liaise with the administration staff regarding recall systems, and to ensure the smooth running of the clinics.
  • To ensure current diabetes templates are accurate for recording the required data for various reporting methods, eg Quality and Outcomes Framework (QOF)
  • To ensure accurate record keeping, recording of all consultations and treatments in the patients computerised medical records using templates where applicable.
  • To maintain accurate data and records in a timely fashion, using SystmOne and other software as applicable.
  • To ensure full understanding of QOF for diabetes chronic disease management and to liaise with the team to ensure targets are achieved.
  • To actively collect and maintain data that will demonstrate patient improvement in key areas.
  • To act as a specialist resource and provide specialist advice in relation to diabetes, to patients their family/carers and health professionals.

  • To play an active role in MDT meetings if required by gathering information and being prepared to update the team on patient progress towards goals etc. (as per their care plan).
