An exciting opportunity has arisen for Newton Place Surgery to employ a motivated and enthusiastic Clinical Pharmacist to join our friendly and supportive team. The role is a part time position.

Newton Place Surgery is part of Mid-Kent PCN. Although we are recruiting as part of the ARRS scheme, you will be employed directly by the practice

Main duties of the job

The post holder is a qualified GPC-registered pharmacist. They will act as part of a multi-professional NHS team at Newton Place Surgery, providing clinical pharmacy services that will protect, maintain and restore the health of patients.

Duties include, undertaking structured medication reviews, providing support and advice to patients with long term conditions, support the practice to achieve QOF and other targets, Contribute to the improvement of health outcomes in longterm conditions as part of the multidisciplinary team and other general tasks as required.

About us

Newton Place Surgery provides primary care services to approximately 19,400 patientsat its purpose-built building in Faversham, Kent.

The practice has been established for a long time and is well respected. The staff members see themselves as an integral part of the community, working with care homes, other local providers and the practices Patient Participation Group. The Partners are forward-thinking and are members of, and have lead roles in, the Faversham Primary Care Network. They are part of the Kent and Medway ICB, with whom they have a strong working relationship.

They are a large practice, having eight Partners and a clinical team of seven salaried GPs, two retainer GPs, a nursing team, allied health professionals such as clinical pharmacists, paramedics, mental health practitioner and a skilled administrative team.

They are an experienced training practice and provide training and development for medical students, doctors at all stages of their training and other allied health professionals.

They value high clinical standards and aim for an excellent patient experience; they have high QoF scores and CQC Good in all areas.

Job responsibilities

Job Summary:

The post holder is a GPC-registered pharmacist who has been qualified for a minimum of 2 years. They will act as part of a multi-professional NHS team at Newton Place Surgery, providing clinical pharmacy services that will protect, maintain and restore the health of patients, always working within their scope of practice.

The role of the post holder will develop over time, in accordance with local patient, service and practice need, but is likely to include:

  • assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team;
  • manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action;
  • use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice;
  • actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person;
  • complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
  • Taking responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex poly-pharmacy
  • To link in with community based pharmacists, medicines management team and KCHFT pharmacy / prescribing teams.
  • Act within a leadership capacity for the prescriptions team and pharmacy technician.
  • Working across the PCN sites when necessary.
  • Undertake prescribing projects for the practice / PCN.
  • Providing primary support to general practice staff with regards to prescription and medication queries. This will include supporting the repeat prescription system, dealing with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).
  • Providing clinical leadership on medicines optimisation and quality improvement, managing some aspects of the quality and outcomes framework and enhanced services.
  • Ensuring that the practice integrates effectively with community and hospital pharmacy services to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
