Nurse Practitioner
Reporting to: GP Partners
and the Practice Manager on organisational and administrative issues
Accountable
to: GP Partners
Hours of work: We are looking for
approximately 20 hours per week
Days of work: Wednesday, Thursday, Friday
Contract type: Part time Advanced
Nurse Practitioner position
Role summary: Provide
assessment,
treatment and diagnosis at first point of contact dealing with a wide range of
health needs in a primary care setting. Comply with the practices General
Medical Services (GMS) contract, DES/LES contracts and adhere to the principles
of good medical practice.

Medical Centre:
We
are a practice of 7500 patients situated in the Boughton Heath area of Chester.
We have a clinical team of GPs, ANPs and Practice Nurses delivering high
quality health care and attaining high QOF achievements. The practice is one of
four in the Chester East Primary Care Network (PCN). We work closely with our
PCN colleagues and are further developing our workforce as a group. The PCN has
recently employed a team of clinical pharmacists and a Pharmacy Technician. We
also have a low-level Mental Health service and a Social Prescribing Link
worker.
The successful candidate will have:
- Experience in General
Practice
- Have an active
registration with the NMC
- Practice within the four
pillars: Advanced Clinical Practice, Leadership, Facilitation of
education and learning, Evidence research and development
- Have a Job Plan that
demonstrates advanced nursing practice and has equity with peers working
at this level
- Be educated to Masters
level l(180
level 7 points (Scottish equivalent of level 11)).
- Be an independent
prescriber
- Meet NMC revalidation
requirements
- Demonstrate autonomous
evidence
Clinical responsibilities:
You will be required to:
Act upon most alerts (e.g. QOF) and reminders during a routine
consultation
Provide all ANP services required
Respond to medical problems
presented by patients, including history taking, examination, investigation,
diagnosis, treatment and referral where appropriate.
Provide appropriate
preventative health care and advice.
Provide medical support as
required to patients referred by nursing staff
Adhere to the practice
prescribing formulary, local and national guidelines.
Make professional, autonomous decisions in relation to presenting
problems, whether self-referred or referred from other health care workers
within the organisation
Record clear and contemporaneous I.T. based consultation notes to
agreed standards
Collect data for audit purposes
Compile and issue computer-generated acute and repeat
prescriptions
In
general the post-holder will be expected to undertake all the normal duties and
responsibilities associated with working within primary care.
Other
responsibilities with the Practice:
Supporting the practice staff
and responding to requests for advice and assistance from the practice
reception, secretarial and nursing staff.
Attend MDT, clinical and
practice meetings.
Provide additional enhanced services to the practices that meet
the needs of the practices patient population and maximise the income streams
to the practice
Awareness of and compliance with all relevant practice
policies/guidelines, e.g. prescribing, confidentiality, data protection, health
and safety, annual QOF Assessment of the Practice.

practice level maintaining a record of their own personal and/or professional
development
Taking responsibility for own development, learning and
performance and demonstrating skills and activities to others who are
undertaking similar work.

The post-holder should recognise the
importance of effective communication within the team and will strive to:

Recognise peoples needs for alternative methods of communication
and respond accordingly
Contribution to the planning and implementation of services
The
post-holder will:
Apply practice policies, standards and guidance
Discuss with other members of the team how the policies, standards
and guidelines will affect own work
Work with the Partners and Management to achieve standards of
quality, performance standards, budgets and targets without compromising levels
of patient healthcare.
Contribute towards the development and implementation of new
standards, policies and procedures that are/will be required of GP Practices
now and in the future (as directed by NHS/ DoH/ SHA/ PCT, new legislation etc.)
