Bramhall & Shaw Heath Medical Group are looking for a Practice Nurse to join our dedicated nursing team to ensure the delivery of high-quality healthcare services to our community.
Main duties of the job
To be fully proficient in chronic disease management, hypertension/BP monitoring, diabetes, respiratory, CVD, vaccinations and immunisations, cervical screening and contraception review.
To provide support to the nursing team including healthcare assistants.
You will partake in the responsibility for overseeing infection control standards and cold chain compliancy.

Job responsibilities
- Provide chronic disease management for Diabetes, COPD, Asthma and CHD/CVD
- Perform cervical screening
- Administer adult and paediatric injections and immunisations
- Undertake venepuncture and electrocardiography
- Provide advice on lifestyle issues, assisting and supporting patients in changing their unhealthy behaviors and refer appropriately
- Deliver travel health care including vaccinations and advice for patients
- Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviors, suicidal tendencies and be aware of local referral procedures
- Work with safeguarding teams where relevant
- Participate in annual appraisal
- Attend clinical and practice meetings
- Ensure that current evidence-based approaches to patient care are standard practice
- Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluid spillages and report and treat sharps injuries
- Support first aid and management of emergencies, e.g burns, haemorrhages, hypo/hyperglycaemic emergencies and asthma attacks
