An exiting opportunity has arisen to share your care of diabetes by overseeing our diabetes patient register and arranging reviews and care plans by delivering excellent tailored care to our patients.
We are looking for a diabetes nurse to
be responsible for the delivery of diabetic nursing services, working as part
of the practice multidisciplinary team, delivering care within their scope of
practice to the entitled patient population. The diabetes nurse will be
responsible for the diabetic register, diabetic care plans and management, diabetic
health prevention and monitoring, as well as supporting the management team in
the reviewing of clinical policies, procedures and patient care initiatives.
We are ideally looking to employ a diabetes nurse with Primary care experience although this is not essential. The candidate would be experienced in diabetes care, able to workindependently, have an understanding of clinical risk management, care planning and strong communication and IT skills.
Main duties of the job
To be responsible for the care of the practice diabetic patients.
Provide care planning, management, reviews and monitoring to our entitled patients.
Manage blood results and initiate patient follow up care.
Direct patients to other services as required.
Work as part of an MDT team.
Liaise with practice staff to meet targets ensuring patient care is prioritised.
Work in line with protocols and procedures
Record consultations, results and plans into the patient elctronic record
About us
Bedford Street Surgery is a two site practice located in Bletchley, Milton Keynes with a branch site in Furzton, Milton Keynes. We currently have over 15000 patients across both sites.
We have a well established clinical MDT team consisting of paramedics, prescribing nurse practitioners, respiratory nurse, health care assistants, practice nurses and gps who are supported by our management and admin team of secretaries, care navigators and data quality.

Primary Responsibilities:
- Develop, implement and embed health promotion
and well-being programmes for diabetes. Assist with the organisation and coordination of the provision of nursing services for the practice population with a diagnosis of diabetes.
- Provide nursing treatments to patients in participation with general practitioners or independently agreed protocols associated with diabetes.
- Provide nursing treatments to patients in
participation with general practitioners or independently agreed protocols
associated with diabetes.
- Provide general and specific health screening
to the Practice patients (within the agreed protocols) with referral to GP as
necessary.
- Supervision of patients with diabetes and those
with additional medical conditions.
- Advise patients on diabetes and general
healthcare with referral to GPs as necessary.
- Undertake care planning for patients with a
diagnosis of diabetes.
- Work with the Practice to improve the health
and well-being of patients with pre-diabetes and diabetes.
- Support the clinical team in the development of
individual treatment plans for chronic disease patients.
- Identify, manage and support patients as risk
of developing diabetes, preventing adverse effects on the patients health.
- Request pathology services as necessary,
processing and interpreting results as required.
- Provide diabetic clinics, delivering patient
care as necessary, referring patients to secondary / specialist care as required
- Maintain accurate clinical records in
conjunction with extant legislation.
- Ensure read codes are used effectively.
- Maintain diabetic disease registers
- Work with the practice to ensure all QoF and
other targets are met.
Secondary Responsibilities
- Maintain and control items
associated with diabetic care including meters etc.
- Undertake the collection of
pathological specimens including intravenous blood samples etc. Perform
investigatory procedures requested by the GPs (within the scope of
personal experience and training only).
- Participate
in local initiatives to enhance service delivery and patient care
- Support
and participate in shared learning within the practice
- Continually review
clinical practices, responding to national policies and initiatives where appropriate
- Participate in the
review of significant and near-miss events applying a structured approach i.e.
root cause analysis (RCA).
- Maintain professional
registration and adhere to revalidation requirements
- Attend relevant training events to
keep updated with good practice, NICE guidance and national and local
policies.
- Maintain effective
liaison with other agencies and staff concerned with patient care and all other
disciplines within the practice, with appropriate regard to confidentiality
