Job summary

HFP Medical has a vacancy for a link worker / social prescriber at Richford Gate Medical Centre. Our PCN has 5 practices in it and we have a Link Worker based at each practice.

Main duties of the job

Please review the job description attachment. Link worker / social prescribers are an important interface between the clinical team in a GP practice and the "outside world". Many patients have non-medical needs which, if solved or improved, can have a significant positive impact on their wellbeing, physical and mental health. The link workers act as the liaison point to support patients in enabling appropriate non-medical local support. The job is very varied and rewarding and would suit someone who has a passion for helping people. Some relevant experience is necessary but that could be in an NHS role, care home setting, social services, the voluntary or charity sector. We may also consider candidates who have volunteering experience but whose paid employment recently has involved customer service (e.g. retail or hospitality)

Our 150 staff care for 69,000 patients. HFP Medical is a single GP partnership with 24 partners. Our leadership team includes a Managing Partner, Medical Director, Managing director, Head of Nursing, and Head of Pharmacy. The vast majority of staff work on one site only so that patients have a familiar team whichever practice they are registered at.
