Job summary
  Working under the direction of the eHub Manager and the eHub Lead.
Ehub/Patches is a centralised hub for the H&F Partnership, running a programme of Patches administration for the Partnership
Carrying out all day to day admin tasks in relation to the management and actioning of Patches for the H&F Partnership surgeries Any other duties relating to administration in the H&F Partnership as requested and directed by Line Manager.
You will effectively manage telephone queries from patients and make telephone calls and enquiries to hospitals and other establishments. You will carry out administrative related urgent and routine tasks as delegated. Support Reception when required and cover other admin areas.
  Main duties of the job
  Responsibilities:
 Provide an administrative service to the eHub and working flexibly with colleagues.
 Effectively communicate with GPs and Sites to provide an efficient running of the eHub to facilitate the delivery of patient care.
 Provide a co-ordinated response to patients submissions of Patches.
 Receive, triage and concisely record Patches that will result in onward management of the eConsult.
 Triaging all Patches received for each Site (rota based)
 Triaging at Site, the same day for site GP/Admin and eHub GP
 Placing eConsults on each GP list as required.
 Dealing with and processing all admin resulting from Patches.
 Booking all Routine appointments and Telephone Consultations arising out of Patches
 Booking straight into face to face appointments as necessary and/or as directed by GP
 Maintaining up-to-date and accurate records of each patient contact.
 Taking instructions and actioning tasks assigned by GPs and/or Line Manager.
