Job summary
This role is a varied multi-faceted
non-clinical role within the Ageing Well Team.
An administrative workload, this role will be responsible for the
co-ordination of administrative duties and workflow. The role requires a high
level of organisation and communication skills and ability to work accurately
and under pressure.
The role will directly support both
the clinical and non-clinical members of the Ageing Well Team in the
organisation of patient's care.
Please note the role is to cover for maternity leave and it is envisaged the post will be for 9 months initially.

Main duties of the job
Co-ordinate
and organise monthly reviews in collaboration with care coordinators,
clinicians, care homes and any external services as required.
Maintain
accurate and timely patient information.
Collate data
and run searches and reports as required for the Ageing Well Team.
To work effectively as part of a team
to provide cover within the Ageing Well Team when required and to be flexible
regarding working hours to meet the needs of the service.
Obtain relevant information from the
team to undertake the administrative tasks as outlined below.

Care Network (PCN) in Chesterfield and Dronfield. Arc Primary Care is an
alliance of GP Practices. Our members consist of 10 GP practices which cover a population

The Ageing Well Service is a local NHS service, operating on
behalf of and as an extension of the usual registered GP practice, who remain
responsible for certain elements of patient care. Our service was established
(and has been evidenced) to improve access to timely, high quality and holistic
(person-centred) care for both care home and house bound patients, as well as
improving continuity of care through provision of dedicated care home
clinicians

ADMINISTRATIVE TASKS
Coordinate
with healthcare providers and staff to achieve QOF and other national/local
health targets.
Undertake
searches and reports from the database in relation to QOF and carry out any
necessary actions in a timely basis co-ordinating meetings with multiple
agencies to attend.
Scanning
and coding of all medical correspondence received into the Ageing Well
Team.
Updating
of patient records electronically as necessary and when updates are
received.
Keep
patients up to date with record updates.
Support
admin staff, providing cover during staff absences.
Managing
your time and workload efficiently to ensure efficient running of the
service.
Handle
general administrative tasks such as filing, scanning and processing
medical correspondence.
Attending
training and staff meetings as required.
Proficiently
use SystmOne/EMIS clinical software for managing patient records, appointments,
and clinical data.
Ensure
patient information is accurately entered and maintained, adhering to data
protection regulations (GDPR).
Provide
support to clinical staff in navigating and troubleshooting SystmOne/EMIS.
Data Input - Scanning and coding of all clinical
and other correspondence received in the practice, including dealing with any
actions marked on the correspondence by a clinician following appropriate protocol.
To oversee the day to day running of the
administrative work undertaken to ensure efficient workflows and provide
support as required.
Support
the team in the use of the clinical systems.
Provide
guidance in trouble shooting when required.
Keep
up to date with, and understanding of, NHS Targets (QOF, Enhanced Services
etc.) and practice performance metrics.
Excellent
