Job overview

To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Main duties of the job

  • Please see attached job description which includes:
  • Usual practice Nurse /treatment room skills - wound care, smears, immunisations etc
  • Monitoring and management of specified long term conditions such as: asthma, chronic obstructive pulmonary disease, rheumatoid arthritis, diabetes, dementia, cardiovascular disease and hypertension.
  • A duty to advise senior nurses of potential problems or errors within a range of assigned tasks
  • Attend and participate in any Practice meetings when required.

Flexible hours up to full time - Monday to Friday Salary - commensurate with experience

We are a large three site practice in Bracknell/Crowthorne. Clinical staff work across all three sites. We are a member practice of The Health Triangle PCN

We pride ourselves on our use of information technology and we have a team of motivated and experienced administration and management staff.

We are a GMS contract holder and use EMIS Web, EPS electronic prescribing and Docman, and we have a high QOF achievement and a GOOD CQC status.
