Job summary

Hollyoaks Medical Centre is looking to recruit an enthusiastic Practice Pharmacist to join our friendly team.

Main duties of the job

The ideal candidate would be able to provide the following services:

To work alongside the PCN pharmacist on the Clinical Excellence and Investment Framework (CEIF) contract.

Be first point of call for any medication queries within the Practice.

Review and screen discharge letters to ensure the patients are being prescribed the correct medication.

To conduct medication reviews on patients.

To carry out medication switches in line with local/national guidelines.

Manage medicines shortages by suggesting suitable alternatives

Provide clinical staff with guidance and updates on medication issues as they become known.

Essential

  • Pharmacy degree.
  • Registered with the GphC.

  • Primary Care experience.
