Job summary

We are seeking a dynamic and enthusiastic newly qualified GP to join our multidisciplinary team. This role has been designed to support our multidisciplinary practice team to provide essential support across key service areas.

The post-holder will contribute to the delivery of high-quality, patient-centered care with a strong focus on Urgent Care, Home Visiting, and Care Home Support, while also contributing to QOF and contractual obligations. The ideal will candidate will be within 2 years post-CCT. The post is likely to be 6-8 sessions.

Main duties of the job

Key Duties and Responsibilities

Urgent Care and On-Call

Provide same-day clinical consultations for urgent presentations

Participate in the on call rota

Work collaboratively to ensure safe and timely patient care

Home Visiting

Undertake same-day or pre-planned home visits

Liaise with community and multidisciplinary teams

Contribute to end-of-life care planning and prescribing

Care Homes

Provide medical support to allocated care homes

Attend regular ward rounds

Build effective relationships

QOF and Contractual Target Delivery

Support the practice in delivering QOF targets

Engage in chronic disease management clinics

Contribute to clinical coding and data quality improvement initiatives

About us

The Middlewood Partnership is a group of four practices across a Primary Care Network footprint which was established following a merger in 2019. We have created a modern GP organisation that retains the benefits of family medicine, including continuity, teamwork and support, but with the benefits of working at scale. Our size allows us to be more resilient and efficient, enabling us to invest in ongoing quality improvement, enhanced care, new services, and the training and development of our workforce.

Middlewood operates a multi-disciplinary service model that includes a robust triage and urgent care system, Long Term Condition clinics, specialist clinics, Enhanced Access services, prescribing and recall services.

With a total list size of 34,000 patients, and care and support collectively provided by 160 members of staff from four locations in three towns in Eastern Cheshire, we are now looking to increase our GP capacity to support the delivery of our Enhanced Access services.
