Job summary

Due to the expansion of our clinical team, we have a unique opportunity for the right individual to join the Middlewood Partnership in this exciting leadership role. We believe there are very few roles where you can feel so involved in really making a difference by developing and leading a multi-disciplinary team of health professionals, being part of the amazing staff team and genuinely making a difference to people's lives.

This opportunity will combine leading and overseeing the delivery of clinical services within a large practice. We believe we have a transformed organisation, delivering primary care at scale whilst maintaining continuity and person-centred care.

This is a leadership role, and the successful individual will be working alongside our clinical leads to develop and establish clinical pathways and protocols. We strive to deliver care that enhances patient experience whilst improving organisational effectiveness.

Our clinical services are delivered from four practice bases in three villages. The successful post holder will be responsible for the management of:

Medicines management team

Health and Wellbeing team

Middlewood Immediate and Necessary Team

Care home and housebound support team

GPs and GP-led services at each of our practice bases

Oversight and co-management of specialist care services such as heart failure and minor surgery clinics

Main duties of the job

Lead and manage the individual services in partnership with clinical leads

Ensure all clinical services are effectively managed and co-ordinated.

Work with clinical leads to develop and establish clinical pathways and protocols

Actively contribute to, and lead clinical service meetings

Provide reports and updates to partnership board

Maintain and improve clinical performance standards

Implement any changes based on service/organisational requirements.

Ensure clinical staff are compliant with policies, procedures as well as statutory and mandatory training

Undertake appraisals for own direct line reports

Review the workforce in line with business requirements to ensure a resilient and sustainable workforce

Ensure effective communication and involvement arrangements are in place for clinical teams

Lead on the recruitment/retention of staff

Ensure that working arrangements are in place to enable clinical staff to comply with Middlewood incident & complaints policies and processes

Investigate complaints/incidents as appropriate

Ensure processes are in place for sharing learning and support from incidents and complaints with all clinical staff

ACCOUNTABLE TO: The Managing Director/Medical Director

RESPONSIBLE FOR:

Provision of managerial function to all clinical staff (apart from nurses) employed within the Partnership. This will include all ARRS staff.

In partnership with lead clinicians, oversee the provision of comprehensive, high quality, patient centred services that meet all requirements of the PCN DES as well as core and enhanced GMS contracted services.

Providing operational management support to clinical service including the delivery of performance, quality and workforce requirements.

Supporting the development and delivery of service strategy, staff engagement, organisational development and the management of risks in partnership with clinical leads.

In partnership with clinical leads ensuring all clinical services meet the CQC requirements to be safe, effective, caring, responsive to needs and are well led.

JOB SUMMARY:

Following the merger of four of our individual practices in April 2019, we believe we now have in place a transformed organisation, delivering primary care at scale, whilst maintaining continuity and person-centred care.

The successful individual will be working closely with our clinical partners to support the delivery of high quality, safe and effective clinical services.

We are looking for someone who has exceptional communication and relationship building skills and who can quickly establish rapport and build trust and respect within a complex organisation.

The main business of the organisation is managed within three functions: operational management, assurance and clinical services. The successful candidate is expected to have management oversight for all our clinical services (apart from nursing).

Our clinical services are delivered from four practice bases in three villages. The successful post holder will be responsible for the management of the following teams:

Medicines management team: comprising of 2 clinical pharmacists, 5 pharmacy technicians, 3 prescribing support administrators.

Health and well being team: comprising of 2 Mental Health Practitioners, 3 social prescribers.

Middlewood Immediate and Necessary Team: comprising 5 Physician Associates, 2 ANP

Care home and housebound support team: comprising 1 paramedic, 1 Physician Associate and 1 ACP. This team works closely with our community team.

GPs and GP-led services (ie. GP Sessions) at each of our practice bases

MAIN TASKS AND RESPONSIBILITIES:

Responsibility for clinical services:

Lead and manage the individual services in partnership with clinical leads whilst sustaining and contributing to effective working relationship with all other managers and clinicians.

Ensuring all clinical services are effectively managed and co-ordinated.

Working with clinical leads to develop and establish clinical pathways and protocols. We strive to deliver care that enhances patient experience whilst improving organisational effectiveness.

Actively contribute to, and lead clinical service meetings, working co-operatively with clinical leads.

Provide reports and updates in a timely manner to partnership board.

Look for new and innovative ways to improve service delivery to make the most effective use of resources whilst ensuring services meet the organisations contractual requirements.

Maintain and improve clinical performance standards by working with our systems team.

In partnership with clinical leads implement any changes based on service and organisational requirements.

Responsibilities for Human Resources;

Line manages the clinical teams described earlier

Lead, manage, motivate and develop staff to maintain the highest level of staff morale and to create a culture within the service that is evidence by high standards as well as openness and transparency.

Support the Medical Director and other clinical leads in the delivery of the annual appraisal process for all Salaried GPs.

Ensure that all clinical staff are compliant with policies, procedures as well as statutory and mandatory training.

Undertake appraisals for own direct line reports and ensure personal development plans are completed and implemented.

Review the workforce in line with business requirements to ensure a resilient and sustainable workforce.

Ensure effective communication and involvement arrangements are in place for clinical teams.
