Job summary
In this role you will work as part of a
prescribing team, comprising Pharmacy Technicians, and Prescribing Support Administrators
who work alongside the Clinical Pharmacist and wider practice team and help
implement our integrated model of prescribing support. The successful candidate
will share our values and have the energy, skills and enthusiasm to contribute
to shaping how we support our patients.

You will lead and support the team to ensure a
coordinated, timely approach to the safe processing of repeat medication
requests, issuing prescriptions following specific checks (which may include
reviewing blood results, checking patients INR, checking any Quality Outcome
Framework and Investment and Impact coding is accurate and up to date) and
dealing with patient requests.
You can expect to receive hospital discharges,
hospital letters and medication recommendations for review and action. You will inform patients when blood tests/ BP
checks/ medication reviews are due and work within our chronic disease
management pathways to keep their electronic notes up to date accordingly.
As team lead, you will provide the day-to-day
line management for
the team and will therefore need to be an excellent communicator and able to
command the respect and trust of the team.

Leadership
- First-line management of the Prescribing team
- Support new starters with induction, training and mentoring
- Oversee the staffing of the team ensuring enough capacity to manage the workload over the working week
- Liaise with clinicians, managers and team leads
- Deal with any immediate responses to complaints relating to prescription requests, escalating to the Clinical Pharmacist and Practice Operational Manager as appropriate
- Implement any changes required by management instructions
- Continually assess and evaluate prescribing systems, recommending changes and improvements to the management team as appropriate
- Organise and manage prescribing team staff meetings in association with the Clinical Pharmacist
- Deal with staff issues such as return to work meetings, performance management, coaching and training, disciplinary issues etc.
- Identify team training needs
- Handle difficult situations in a calm and professional manner
- Prioritise the teams workload
Administration
- Safe processing of repeat medication requests
- Help to achieve success in prescribing incentive schemes
- Transfer medicines information from other healthcare agencies onto in-house patient information systems
- Communicate with local pharmacies to ensure efficient & well-managed repeat prescribing system
- Liaise with practice doctors and staff over prescribing issues
- Liaise with Cheshire and Merseyside ICB over prescribing policies, targets and general issues
- Liaise with nursing and residential homes in the practice area re medication requirements; provide and monitor their scripts.

Policy and service development
- Liaise with Cheshire and Merseyside ICB re. local prescribing policies
- Alongside the Clinical Pharmacist and GP Prescribing Lead, define and create protocols and formularies within the practice in consultation with other members of the team including the ICB medicine management team
- Working as a clinical co-ordinator, ensure success in gaining Quality Outcome Framework and Investment and Impact Fund points
- Work autonomously with due attention to agreed protocols and practice
- Prioritise workload effectively

- Contact patients after drug alerts
