The Middlewood Partnership

This job is now closed

Job summary

The Advanced Nurse Practitioner is a key member of the Middlewood Partnership Primary Care Network multidisciplinary team. We work with an online contact/booking system, plus telephone support for those who are not able to use IT. At the point of triage, we prioritise, direct and allocate the query to the right team or person; that may be within our organisation or in the wider health and care system. The ANP would be a crucial part of our clinical assessment team, undertaking face-to-face consultations in urgent care/minor illness clinics, and in patient’s own homes.

Main duties of the job

The post holder, acting within their scope of practice, will:

Work within our clinical allocation hub to triage patient enquiries and requests.

Guide, signpost and move patients through our system to ensure they see the right professional in a timely manner.

Offer expert advice via telephone and face to face assessment on a range of minor illness and minor ailments, including more complex case management, as appropriate, within scope of practice.

Undertake home visits and clinical assessments as part of our visiting service. This will be with support and guidance from our visiting GP and working as part of an integrated multidisciplinary team

About us

If you’re interested in working for a committed, responsive practice that values every member of its team, then the Middlewood Partnership is for you. We’re a single practice Primary Care Network (PCN) – that makes us one multi-disciplinary team who all work together, contributing our particular skills and experience, to provide the best possible care to our 34,000 patients across the villages of Bollington, Disley and Poynton.
