The Middlewood Partnership is an innovative practice that
forms a single Primary Care Network, providing a solid foundation for
place-based care in Eastern Cheshire. Our strong principles of high-quality
care, mutual respect and shared values has helped develop a bold and ambitious
shared vision for our practice. Our team of
150 staff support and care for 34,000 patients across four locations in three
towns in Eastern Cheshire.
We are now looking to increase our salaried
GP sessions. This is a unique opportunity to be part of an innovative, forward
thinking general practice that offers GPs opportunities for:
Job variety – We offer opportunities to provide routine
general medical care, speciality care in chronic disease management, women’s
health, research, education and supervision, care home support, medicines
management.
Clinical support – Our clinical model enables team working and clinical support where on
a regular basis you are co-located alongside other GPs Physician Associates,
Paramedic and Advanced Nurse Practitioner
True multi-disciplinary working - We’ve
embraced the opportunity to include new roles in our varied and expanding
multi-disciplinary team. We have a well-established team of clinicians
including: salaried GPs, Advanced Nurse Practitioners, Diabetes Specialist
Nurses, Tissue Viability Nurse, Clinical Pharmacists and Pharmacy Technicians, Physician
Associates. Paramedic. Social Prescribers.
Integrated working - We work very closely
with our community team and have established a shared visiting service with our
district nurses. In addition, we have in
place a digital triage process where all clinical requests are triaged by a
clinical team based in our Clinical Hub
Professional development – We
encourage our salaried GPs to personally and professionally develop. Examples of this are:
o One of our salaried GPs chairs or staff well-being group
o A salaried GP is our prescribing lead
o Recently one of our salaried GPs has been supported to undertake training
to run women’s health clinics
Education and training – We are a
training practice for GP registrars as well as having regular medical
students. In addition, we offer regular
placements for PA students. We currently
have a GP fellow for 12 months who is focusing on education.
Research – We are a level 3 NIHR site and regularly run research studies that are relevant and
beneficial to our patients. One of our
salaried GPs joined us following a 12-month research fellowship and is now
working very closely with our Research Lead Partner.

Well-being – General Practice can be a challenging
environment to work in at present. We
aim for job plans to have variety to support well-being. We have a staff well-being group who are active
in running events and supporting our teams.
The
post-holder will be an important member of our clinical team and will provide
high quality General Medical Services including dealing with a wide range of
health needs in a primary care setting, ensuring the highest standards of care
for all registered and temporary patients.
Clinical
Responsibilities:
- Undertake a
variety of duties including face to face consultations, telephone and
online consultations and queries, visiting patients at home, checking and
signing repeat prescriptions and dealing with queries, paperwork and
correspondence in a timely fashion
- If required
support the Middlewood Prescribing Team with medication queries
When required supporting the Middlewood Clinical Hub which
facilitates digital triage of all clinical queries.
- Making
professional, autonomous decisions in relation to presenting problems,
whether self-referred or referred from other health care workers within
the organisation
- Assessing the
health care needs of patients with undifferentiated and undiagnosed
problems
- Screening
patients for disease risk factors and early signs of illness
- In consultation
with patients and in line with current practice disease management protocols,
developing care plans for health and social well-being
- Recording clear
and contemporaneous consultation notes to agreed standards
- Collecting data
for audit purposes
- Compiling and
issuing computer-generated acute and repeat prescriptions (avoiding
hand-written prescriptions whenever possible)
- In general the
post-holder will be expected to undertake all the normal duties and
responsibilities associated with a GP working within primary care.
Other
Responsibilities within the Organisation:
- Awareness of
and compliance with all relevant practice policies/guidelines, e.g.
prescribing, confidentiality, data protection, health and safety
- A commitment to
life-long learning and audit to ensure evidence-based best practice
- Contributing to
evaluation/audit and clinical standard setting within the organisation
- Maintain
patients’ clinical records in line with the practice patient record
system.
- Attending
training and events organised by the practice or other agencies, where
appropriate.
- Attend team
meetings and other meetings as may be necessary
To work within a team setting, with flexibility
and due care for all members of staff.
