Roles and Responsibilities
- Oversee and provide proficient clinical assessment, screening, health education and clinical care to patients ensuring clinical nursing practice is patient centred and evidence-based
- Utilise and demonstrate sensitive and appropriate communication styles,
- Maintain accurate, contemporaneous and complete records of patient consultations, consistent with legislation, policies and procedures.
- Use effective time-management strategies to prioritise, organise and manage own workload in a manner that maintains and promotes quality
- Participate in the delivery of scheduled immunisation and vaccination programmes for both adults and children, as appropriate.
- To promote and develop nursing excellence in women’s health, with particular reference to safe cervical screening processes, nurse-led sexual health initiatives, contraception and HRT programmes, as appropriate
· Work in conjunction with our Middlewood TVN and the wider nursing team to promote and deliver excellence in wound care
- Be responsible for ensuring chronic disease management programmes are delivered in a safe effective and efficient manner, working with recall teams to ensure timely and effective reviews meet QOF requirements.
- Oversee and ensure specific programmes are in place for the screening and nursing management of patients with respiratory and cardiovascular conditions:- Florey remote assessments and face to face consultations- Asthma and COPD, +/- spirometry- Cardiovascular risk assessment and management of cholesterol and hypertension
- Help identify and effectively manage patients with pre-diabetes, ensuring timely interventions to promote health and well-being
- Support and develop effective healthy eating and weight management programmes for our practice population
- Develop and enhance opportunities for nurses to participate in minor illness and ailment pathways with support of the GP team
- To establish innovative ways of working with the wider multi-disciplinary team and care community
