Job summary

We’re looking to recruit a Senior Practice Nurse who can lead, support and be an excellent role model for our nursing team. If you’re an enthusiastic, forward-thinking General Practice Nurse with appropriate clinical skills, knowledge and experience seeking a role where you can really make a difference, we’d love to hear from you.

  • Oversee & provide proficient clinical assessment, screening, health education and clinical care to patients
  • Utilise sensitive & appropriate communication styles
  • Maintain accurate, complete records 
  • Use effective time-management strategies 
  • Participate in the delivery of scheduled immunisation & vaccination programmes 
  • Promote & develop nursing excellence in women’s health
  • Work in conjunction with our Middlewood TVN to deliver excellence in wound care
  • Be responsible for ensuring chronic disease management programmes are delivered safely & effectively
  • Oversee and ensure specific programmes are in place for the screening & nursing management of patients with respiratory & cardiovascular conditions
  • Help identify & effectively manage patients with pre-diabetes
  • Support & develop effective healthy eating and weight management programmes 
  • Develop & enhance opportunities for nurses to participate in minor illness & ailment pathways 
  • Establish innovative ways of working with the wider multi-disciplinary team & care community
  • Identify and promote new & innovative strategies to supportMental health & Children/Young People’s access to services

Roles and Responsibilities

  • Oversee and provide proficient clinical assessment, screening, health education and clinical care to patients ensuring clinical nursing practice is patient centred and evidence-based
  • Utilise and demonstrate sensitive and appropriate communication styles,
  • Maintain accurate, contemporaneous and complete records of patient consultations, consistent with legislation, policies and procedures.
  • Use effective time-management strategies to prioritise, organise and manage own workload in a manner that maintains and promotes quality
  • Participate in the delivery of scheduled immunisation and vaccination programmes for both adults and children, as appropriate.
  • To promote and develop nursing excellence in women’s health, with particular reference to safe cervical screening processes, nurse-led sexual health initiatives, contraception and HRT programmes, as appropriate

· Work in conjunction with our Middlewood TVN and the wider nursing team to promote and deliver excellence in wound care

  • Be responsible for ensuring chronic disease management programmes are delivered in a safe effective and efficient manner, working with recall teams to ensure timely and effective reviews meet QOF requirements.
  • Oversee and ensure specific programmes are in place for the screening and nursing management of patients with respiratory and cardiovascular conditions:- Florey remote assessments and face to face consultations- Asthma and COPD, +/- spirometry- Cardiovascular risk assessment and management of cholesterol and hypertension
  • Help identify and effectively manage patients with pre-diabetes, ensuring timely interventions to promote health and well-being
  • Support and develop effective healthy eating and weight management programmes for our practice population
  • Develop and enhance opportunities for nurses to participate in minor illness and ailment pathways with support of the GP team
  • To establish innovative ways of working with the wider multi-disciplinary team and care community
  • Identify and promote new and innovative strategies to support- Mental health- Children and Young People’s access to services

  • NMC Registered General Nurse
  • Appropriate Qualifications/Diplomas/Certificates for any declared special interests

Experience

Essential

  • Minimum of three years in a General Practice Nursing role
  • Evidence of continuing professional development pertinent to Primary Care Nursing
  • Evidence of working in a supervisory capacity/ Team Leader role

Desirable

  • Proficient in core practice nursing skills
  • Experience of using EMIS web
  • Experience of using other Online systems, eg AskMyGP, AccuRx, remote consultation, MS Teams

Knowledge and Skills
