Job summary

The Middlewood Partnership is a GP Practice and Primary Care Network of 33,500 patients, established in April 2019. We operate from 4 sites across Bollington, Disley and Poynton and we are looking for an enthusiastic individual who shares our values and has the energy and skills contribute to shaping how we deliver care for the future, to join the Middlewood Partnership as a Social Prescribing Link Worker with a specialist interest in health and wellbeing.

It is anticipated that interviews will be held on Tuesday 7/9/21

Social prescribing is a way of linking people to sources of support within the community. It provides non-medical focused options that can operate alongside existing treatments or care packages to improve health and well-being. Social prescribing empowers people to take control through referral to link workers who give time, focus on what matters to them and take a holistic approach to an individuals health and wellbeing. They support people to make positive lifestyle changes and connect them to diverse community groups and statutory services for practical and emotional support.

Be an active part of The Middlewood Hub and Team BDP, a multidisciplinary team supporting the Middlewood Care Model, receiving, managing and prioritizing a case-load of patient referrals that meet the criteria for health and wellbeing coaching or social prescribing.

Ensure suitability of referrals and follow up with one-to-one contact with the patient to discuss the referral, making home visits where appropriate within organisations policies and procedures.

Develop trusting relationships, giving people time to tell their stories and focus on what matters to them.

Build trust and respect with the person, providing non-judgemental and non-discriminatory support, respecting diversity and lifestyle choices.

Identify the right level of support for the referral, between coaching support and social prescribing.

Help patients to identify the wider issues that impact on their health and wellbeing.

Work with individuals to co-produce a simple personalised support plan to address their health and wellbeing needs based on the persons priorities, interests, values, cultural and religious/faith needs and motivations including what they can expect from the groups, activities and services they are being connected to.

Empower patients to manage their own wellbeing and live independently.

Support patients to work through their personalised support plan, connecting with them at agreed intervals to provide continued support towards achieving their goals, providing coaching and motivating patients to achieve their objectives.

Evaluate how the actions in the care and support plan are meeting the patients health and wellbeing needs.

Where appropriate, physically introduce people to appropriate community groups, activities and statutory services, ensuring they are comfortable, feel valued and respected. Follow up to ensure they are happy, able to engage, included and receiving good support.

Liaise with the referring health professional and seek advice and support to discuss patient-related concerns, referring the patient back to the GP or other suitable health professional if required.

Forge strong links with a wide range of local VCSE organisations, community and neighbourhood level groups, utilising their networks and building on whats already available to create a menu of diverse community groups and assets, who promote diversity and inclusion.

Explore and support access to a personalised health budget where appropriate.

Be an active part of the local peer support network for Social Prescribers.

Undertake basic health professional, other professional and patient engagement activity to promote the service. Deliver simple training to health professionals about social prescribing and referral processes.

Where required and as appropriate, refer patients back to other health professionals within the Middlewood Partnership and Team BDP care community.

Engage in Supervision sessions as provided by The Middlewood Partnership.
