Job summary

We have an opportunity for a Health Care Assistant to join our newly established Middlewood Partnership, a Primary Care Network of 33,500 patients. This is a unique opportunity to be part of, and contribute to, the development of a new and innovative primary care organisation. Main duties of the job

As a healthcare assistant you will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care. You will work under the direct supervision of the Practice Nurses and strictly in accordance with specific practice guidelines and protocols; training will be provided in line with a personal development plan.

Key Responsibilities

Communication

Communicate effectively with other team members

Communicate effectively with patients and carers, recognising their needs for alternative methods of communication

Clinical

You will undertake, record and follow guidelines for the tasks for which you have received appropriate training:

  • ECG recording

  • Phlebotomy

  • Spirometry (if qualified)

  • Borderline BP patient checks and uncomplicated hypertensive patients

  • Well man checks

  • Anticoagulated patients

  • Immunisations, including influenza, pneumovax and vitamin B12 under PSD

  • Supporting diabetic nurses with patients physical examination duties and foot examination, collection of blood investigations and urine ACR

  • Assist member of the team when required: Chaperoning, Moving and Handling, Patient and offering support. Depending on appropriate training.

Processing and management of laboratory samples requested by GPs/nurses

Preparing and maintaining environments and equipment before, during and after patient care interventions

Assisting GPs during the performance of minor operations (With Training)

Assisting in the assessment and surveillance of patients health and well-being

Undertaking specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual

Helping to raise awareness of health and well-being and how it can be promoted

  • NHS Health checks e.g. testing urine, blood pressure checks, heights, and weights and taking simple document medical history.

  • Dressings (within the scope of the role)

Assisting with the collection and collation of data on needs related to health and well-being

  • Supporting, where appropriate the Chronic Disease Management for Middlewood
  • Supporting the nursing team with stock control and ordering
  • All contacts with patients must be recorded in the electronic patient records ( EMIS).
