Job summary

The Middlewood Partnership is a Primary Care Network of practices that merged on the 1st of April 2019 to establish the Middlewood Partnership. This is a unique opportunity to be part of, and contribute to, the development of a new and innovative primary care organisation. We are looking for a Nurse Practitioner (or Advanced nurse practitioner/Nurse clinician) who will work across our four base practices, as part of our newly established clinical assessment team. Your role will include working within our clinical allocation hub as well as supporting and working within our home visiting team and developing urgent care/minor illness service.

Main duties of the job

We are looking for someone who is experienced in general practice or urgent care and who is enthusiastic, accomplished and self-motivated to join our dynamic team to work in a patient facing role. The successful candidate will have excellent skills in clinical decision making and in proactively managing a high workload that requires prioritisation. Within the clinical triage aspect of the role you will be supported by a team of non -clinical allocators who manage all patient requests and a GP for advice and guidance. We have recently established an integrated visiting service, along with our community trust, and we have appointed to the position of Community Practitioner. We would expect the post holder to work alongside the Community Practitioner in delivering these functions.

Job summary:

To work in the role of Nurse Practitioner as a key member of the Middlewood Partnership Primary Care Network multidisciplinary team. The post holder will be an experienced Practitioner who, acting within their scope of practice, will:

Work within our clinical allocation hub to triage patients based on clinical assessment. In addition we would anticipate that the post holder will be able to offer advice for minor ailments, signposting guidance and move patients through our system to ensure the see the right professional in a timely manner. The successful post holder will also be required to undertake clinical assessments face to face on a cohort of patients within their scope of practice.

Undertake home visits and clinical assessments as part of our visiting service. This will be with support and guidance from our visiting GP and working as part of an integrated multidisciplinary team.

Specific functions will be to:

To provide safe and effective online and telephone advice, face to face health assessments and advice as well as provide health and well-being information and advice to the Middlewood Partnership Patients.

To ensure all online and Telephone assessments are of a standard ensure the most appropriate Clinical outcome for the patient is reached.

To work as part of the multi-disciplinary team in providing quality, evidence-based health care to meet the immediate needs of the patient.

To take an active role within the cross-disciplinary, Multi-Skilled team to ensure service quality in line with our organisational goals and strategy, and to assist in the development of new models for service delivery which have a positive impact on the health care of our patients.

To carry out visits to patients private homes, as well as visits to patients in Nursing / Care homes.

Provide an alternative model to urgent and same day GP home visits for the network.

Undertake clinical audits where required

Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered.

Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Analyse reports in order to support patient care

Ensure the provision of high standard of care at all times

Principal duties and responsibilities:

Online and Telephone Triage and face to face assessments:

To receive online and telephone enquiries from our patients, undertake an assessment either remotely or face to face and ensure appropriate, up to date clinical guidance for presenting concerns is provided and the patient is directed to the most appropriate professional depending on their needs

Give self-care and minor ailment advice as appropriate, referring to other service providers where necessary according to clinical need and closing where appropriate cases as advice.

Deliver appropriate face to face assessments through history, examination, investigation and advice. Management decisions will be supported by appropriate clinical prescribers where medications are required (if needed)

To participate in the evaluation of the service that is offered and implement changes as required.

To keep accurate and immediate records of every assessment and advice given regarding patient care, using our clinical system.

To encourage patients to carry out self-care as appropriate through the effective communication of evidence-based health care advice.

To provide up to date education to patients in the correct use of primary care and emergency care services, and other health care related organisations.

To maintain patient confidentiality.

To ensure cases audited meet the clinical and operational indicators and to ensure continuous development and improvement of a quality service in line with the clinical governance agenda.

To promote effective communication and networking within the team.

Professional and Educational Responsibilities

To work within the professional Code of Conduct and other directives as defined by the Professional Body.

To undertake appropriate personal and professional development to meet the requirements of Professional Registration.
