Main duties of the job

  • Work within a nationally recognised medicines management team (The Pharmaceutical Journal, September 2017).
  • Help to build and deliver Pharmacist-led clinics, utilising your skills and experience to provide our patients with specialist knowledge and expertise to manage their conditions.
  • Provide medication support, advice and guidance to Patients, GP colleagues and the wider MDT.
  • Complete medicines reconciliation following hospital discharge and appointments in secondary care.
  • Take part in education and training (including delivery of learning sessions where appropriate).

About us

Bay Medical Group is proud to be the general practice serving the population of Morecambe and Heysham. We operate out of 5 sites and deliver general medical services to c54,000 patients.

Formed in 2016/17 by established legacy practices in the town, we are fortunate to be a single at scale provider in a distinct geographical area and as such were able to establish the Primary Care Network (PCN) for Morecambe and Heysham in July 2019, with Bay Medical Group as the only Practice in the PCN.

Job Purpose statement

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists, technicians and other members of a medicines management team (MMT).

The post holder will work as part of a multi-disciplinary team in both an administrative and a patient-facing role.

The post holder will undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient.

The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

The post holder will be expected to complete either the Primary care pharmacy education pathway, or MSc Advanced Practice Pathway, and the non-medical prescribing course if not already completed.

  • Working knowledge and understanding of pharmaceutical standards and national guidelines.
  • Advanced IT skills.

Desirable

  • Interest in improving polypharmacy outcomes.

  • Flexibility with working hours/ base to meet the needs of the service and patients.
  • Willingness to travel to educational sessions and to fully partake in development courses and opportunities.

Qualifications

  • Registrant of General Pharmaceutical Council (GPhC) and commitment to CPD.
  • Independent prescriber or willing to work towards this within 3 years of appointment.
  • Completion of either the Primary care pharmacy education pathway, or MSc Advanced Practice Pathway or willing to work towards this within 3 years of experience as a primary care pharmacist.

  • Member of Professional Body e.g. Royal Pharmaceutical Society.
  • Hold post graduate clinical Diploma or equivalent.

  • At least 1 years post qualification experience in General Practice, hospital or community pharmacy.
