Main duties of the job
- Work within a nationally recognised medicines management team (The Pharmaceutical Journal, September 2017).
- Help to build and deliver Pharmacist-led clinics, utilising your skills and experience to provide our patients with specialist knowledge and expertise to manage their conditions.
- Provide medication support, advice and guidance to Patients, GP colleagues and the wider MDT.
- Complete medicines reconciliation following hospital discharge and appointments in secondary care.
- Take part in education and training (including delivery of learning sessions where appropriate).

About us
Bay Medical Group is proud to be the general practice serving the population of Morecambe and Heysham. We operate out of 5 sites and deliver general medical services to c54,000 patients.
Formed in 2016/17 by established legacy practices in the town, we are fortunate to be a single at scale provider in a distinct geographical area and as such were able to establish the Primary Care Network (PCN) for Morecambe and Heysham in July 2019, with Bay Medical Group as the only Practice in the PCN.
