As part of this vacancy we are looking to fill 26 hours a week. The working pattern is Monday 8.30am - 6pm, Wednesday 9am - 5.30pm, Thursday 8.30am - 6pm. This is a temporary vacancy for 9 months to cover maternity leave.

Interviews week commencing 28th February.  

Main duties of the job

Bay Medical Group provides a new model of primary care that is equipped to respond to and embrace the challenges presented by the GP Forward View as well as being actively involved in the local health economy.

We operate over 5 sites in Morecambe delivering GP services to over 54,000 patients and our scale means that our Medicines Management Team works in a unique way, to deliver excellent service to our patients, as highlighted in September 2017 in The Pharmaceutical Journal. We have a team of eight Pharmacists working within the practice all based in the same site in our MMT Hub focusing on supporting colleagues and patients with medication queries, medicines reconciliation following hospital discharge and appointments in secondary care.

We are particularly keen to hear from you if you are:-

  • A Pharmacist who is willing to work within our MMT Hub.
  • An independent prescriber, have clinical skills and are already working in or are keen to start working directly with patients as part of our Unscheduled Care and triage team.

Bay Medical Group is proud to be the general practice serving the population of Morecambe and Heysham. We operate out of 5 sites and deliver general medical services to c54,000 patients.

Formed in 2016/17 by established legacy practices in the town, we are fortunate to be a single at scale provider in a distinct geographical area and as such were able to establish the Primary Care Network (PCN) for Morecambe and Heysham in July 2019, with Bay Medical Group as the only Practice in the PCN.

Job responsibilities

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists, technicians and other members of a medicines management team (MMT).

The post holder will work as part of a multi-disciplinary team in both an administrative and a patient-facing role.

The post holder will undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient.

The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Knowledge

Essential

  • Working knowledge and understanding of pharmaceutical standards and national guidelines
  • Advanced IT skills

Desirable

  • Interest in improving polypharmacy outcomes

Qualifications

Essential

  • Registrant of General Pharmaceutical Council (GPhC) and commitment to CPD.

Desirable

  • Member of Professional Body e.g. Royal Pharmaceutical Society.
  • Hold post graduate clinical Diploma or equivalent.
  • Completion of either the Primary care pharmacy education pathway, or MSc Advanced Practice Pathway or willing to work towards this within 3 years of experience as a primary care pharmacist.
  • Independent prescriber or willing to work towards this within 3 years of appointment.

  • Ability to form and maintain good working relationships within a multi-disciplinary team and work effectively as a team member.
