Job summary
The Station Practice is looking for a Advanced Nurse Practitioner to join our friendly pro-active team.
We are proud to have an extended clinical team of an Advanced Nurse Practitioner, Mental Health Nurse, First Contact Physiotherapists, Pharmacist Practitioners, Paramedics, Practice nurses and Health Care Assistants. You will also be supported by an efficient, friendly reception/administration team.
Main duties of the job
The post holder is an experienced prescribing nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
They will work autonomously seeing patients within their own homes, support the care home ward round once a week as well as consult on-site patients.
They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
In order to work at this level, the candidate must be able to map to HEE and Skills for Health frameworks for advanced nursing practice as an ANP.

Job responsibilities
You will work autonomously, seeing patients within their homes without immediate support and supervision, enabled by a collaborative and supportive working relationship supported by the clinical team. Working independently and in conjunction with other health care professionals acting within their professional boundaries; to provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. Demonstrate safe, clinical decision-making and expert care for patients within the general practice.
You will deliver safe, effective clinical decision-making and expert quality care, including assessment, diagnosis, clinical management and evaluation to the Practice population. You will have the attitude, skills and knowledge base to deliver quality holistic care and treatment.
You will assist in the provision of patient care at the practice premises, or in the patients place of residence, working in collaboration with a multidisciplinary team.
Clinical Responsibilities including:
Provide a first point of contact to assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs
Demonstrate critical thinking in the clinical decision-making process
Manage patient risk effectively and safely, including high levels of complexity and uncertainty
Evaluate the safety and effectiveness of your own clinical practice
Ensure safe and effective patient care, demonstrating awareness of your own limitations supported by the clinical team
Provide safe, evidence-based, cost-effective, individualised patient care within the surgery, patients own home or other environment where patient care is carried out
Refer patients directly to other services/agencies as appropriate, utilising Practice and local guidelines and in a timely manner

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug based treatment methods into a management plan
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, and national and practice protocols, and within own scope of practice
Work with patients in order to support compliance with and adherence to prescribed treatments
Provide information and advice on prescribed and over-the-counter medication on medication regimens, side-effects and interactions
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Support patients to adopt health promotion strategies that promote healthy living and to apply principles of self-care
Recognise, assess and refer patients with mental health within scope of practice
Communicate with and support patients receiving bad news
