Job summary

An Advanced Clinical Practitioner is defined as a registered Health Care Professional; you will be a Nurse Practitioner,who is qualified with an appropriate Masters level qualification relevant to Primary Care. We may also consider other relevant and qualified Advanced Clinical Practitioners. You will deliver safe, effective clinical decision-making and expert quality care, including assessment, diagnosis, clinical management and evaluation to the Practice population. You will have the attitude, skills and knowledge base to deliver quality holistic care and treatment.

Main duties of the job

Clinical Responsibilities including: Provide a first point of contact to assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs Demonstrate critical thinking in the clinical decision-making process Manage patient risk effectively and safely, including high levels of complexity and uncertainty Evaluate the safety and effectiveness of your own clinical practice

About us

The Station Practice is located next to the train station in Hastings which is ideal for easy travel, the practice building is spacious and holds on site security. The practice has 3 partners, 1 ANP, 4 practice nurses and 4 HCAs, a reception and admin team alongside locum support staff.

Essential

  • Registered Nurse (NMC/RCN), Registered Paramedic (HCPC) or Registered Physiotherapist (HCPC)
  • Current registration
  • Independent Prescriber
  • Masters level equivalent degree
  • Minor Illness Diploma or equivalent level of qualification/experience
  • Evidence of Continuing Professional Development (CPD)
  • Minimum 5 years post-registration
  • Management of minor illnesses or ailments
  • Proven ability of the safe management of acute and ongoing illness in patients presenting with undifferentiated diagnoses
  • Able to work autonomously, without immediate support and supervision
  • Practitioner-led triage
  • Proven ability to evaluate the safety and effectiveness of own clinical practice
  • IT skills
  • Experience of Microsoft Office applications.
  • Experience of audit
  • Supervision of students
  • Change management experience and direct involvement of redesign within a service, including evidence based practice
  • Good communication (oral and written) and inter personal skills
  • Change management skills and ability to support patients to change lifestyle
  • Able to undertake the requirements of the post
  • Knowledge of health promotion strategies
