Job summary
We are seeking a qualified and proactive Clinical Pharmacist to join our general practice team. Working within their professional scope of practice, the post holder will collaborate closely with clinicians and other healthcare professionals to enhance patient care.
As a key member of both the practice team and a broader network of pharmacists and multidisciplinary colleagues, the Clinical Pharmacist will play a vital role in ensuring the safe and effective use of medicines. This includes acting as a key link for medicines-related communication between primary care and hospital settings.
The role involves supporting clinicians in the management of patients, resolving medication-related issues, and contributing to safer prescribing practices and improved health outcomes.
The successful candidate will have completed an accredited training pathway, including independent prescribing qualifications, enabling them to prescribe and manage treatment plans autonomously in a primary care environment.
Comprehensive supervision, mentorship, and ongoing professional development support will be provided to ensure the Clinical Pharmacist thrives within the role.

Main Duties of the Clinical Pharmacist Role
The Clinical Pharmacist will play a key role in improving patients health outcomes and enhancing the efficiency of the primary care team. They will provide expert, timely, and accessible medicines support, including direct face-to-face contact with patients.
